x86/xen: split xen_smp_prepare_boot_cpu()
Split xen_smp_prepare_boot_cpu() into xen_pv_smp_prepare_boot_cpu() and
xen_hvm_smp_prepare_boot_cpu() to support further splitting of smp.c.
