MIPS: Usage and cosmetic cleanups of page table bits.
* Clean up white spaces and tabs.
* Get rid of remaining hardcoded values for calculating
shifts and masks.
* Get rid of redundant macro values.
* Do not use page table bits directly in #ifdef's.
