RDMA/bnxt_re: Remove set but not used variables 'pg' and 'idx'
Fixes gcc '-Wunused-but-set-variable' warning:

commit cee0c7bba486 ("RDMA/bnxt_re: Refactor command queue management
code") involved this, but not used.
