scsi: bnx2i: Convert timers to use timer_setup()
In preparation for unconditionally passing the struct timer_list pointer to all timer callbacks, switch to using the new timer_setup() and from_timer() to pass the timer pointer explicitly. There was a seemingly missing call to initialize the timer in one handler, so this was added to remove the open-coded initialization. Cc: QLogic-Storage-Upstream@qlogic.com Cc: "James E.J.
