The dmx3191d driver is not capable of DMA or PDMA so all transfers
use PIO. Now that large slow PIO transfers periodically stop and call
cond_resched(), the max_sectors limit can go away.

The driver has a limit of eight LUs because of the byte-sized bitfield
that is used for busy flags. That means the maximum LUN is 7. The default
is 8.

Now that atari_scsi and sun3_scsi have been converted to use the NCR5380.c
core driver, remove atari_NCR5380.c. Also remove the last vestiges of its
Tagged Command Queueing implementation from the wrapper drivers.
The TCQ support in atari_NCR5380.c is abandoned by this patch. It is not
merged into the remaining core driver because,
1) atari_scsi defines SUPPORT_TAGS but leaves FLAG_TAGGED_QUEUING disabled
by default, which indicates that it is mostly undesirable.
2) I'm told that it doesn't work correctly when enabled.
3) The algorithm does not make use of block layer tags which it will have
to do because scmd->tag is deprecated.
4) sun3_scsi doesn't define SUPPORT_TAGS at all, yet the the SUPPORT_TAGS
macro interacts with the CONFIG_SUN3 macro in 'interesting' ways.
5) Compile-time configuration with macros like SUPPORT_TAGS caused the
configuration space to explode, leading to untestable and unmaintainable
code that is too hard to reason about.
The merge_contiguous_buffers() code is also abandoned. This was unused
by sun3_scsi. Only atari_scsi used it and then only on TT, because only TT
supports scatter/gather. I suspect that the TT would work fine with
ENABLE_CLUSTERING instead. If someone can benchmark the difference then
perhaps the merge_contiguous_buffers() code can be be justified. Until
then we are better off without the extra complexity.

Standardize the DMA setup hooks so that the DMA implementation in
atari_NCR5380.c can be reconciled with pseudo DMA implementation in
NCR5380.c.
Calls to NCR5380_dma_recv_setup() and NCR5380_dma_send_setup() return
a negative value on failure, zero on PDMA transfer success and a positive
byte count for DMA setup success.
This convention is not entirely new, but is now applied consistently.
Also remove a pointless Status Register access: the *phase assignment is
redundant because after NCR5380_transfer_dma() returns control to
NCR5380_information_transfer(), that routine then returns control
to NCR5380_main(), which means *phase is dead.

Those wrapper drivers which use DMA define the REAL_DMA macro and
those which use pseudo DMA define PSEUDO_DMA. These macros need to be
removed for a number of reasons, not least of which is to have drivers
share more code.
Redefine the PDMA send and receive hooks as DMA setup hooks, so that the
DMA code can be shared by all 5380 wrapper drivers. This will help to
reunify the forked core driver.

The io_recovery_delay macro is intended to insert a microsecond delay
between the chip register accesses that begin a DMA operation. This
is reportedly needed for some ISA boards.
Reverse the sense of the macro test so that in the common case,
where no delay is required, drivers need not define the macro.

For those wrapper drivers which only implement Programmed IO, have
NCR5380_dma_xfer_len() evaluate to zero. That allows PDMA to be easily
disabled at run-time and so the PSEUDO_DMA macro is no longer needed.
Also remove the spin counters used for debugging pseudo DMA drivers.

The only chip that needs the workarounds enabled is an early NMOS
device. That means that the common case is to disable them.
Unfortunately the sense of the flag is such that it has to be set
for the common case.
Rename the flag so that zero can be used to mean "no errata workarounds
needed". This simplifies the code.

Both drivers also define NCR5380_dma_xfer_len, which means

If REAL_DMA were to be defined, compilation would fail.
For the atari_NCR5380.c core driver, REAL_DMA is always defined.

Drivers that define PSEUDO_DMA also define NCR5380_dma_xfer_len.
The core driver must call NCR5380_dma_xfer_len which means
FLAG_NO_PSEUDO_DMA can be eradicated from the core driver.
dmx3191d doesn't define PSEUDO_DMA and has no use for FLAG_NO_PSEUDO_DMA,
so remove it there also.

This change brings a number of improvements: fewer macros, better test
coverage, simpler code and sane Kconfig options. The downside is a small
chance of incompatibility (which seems unavoidable).
CONFIG_SCSI_GENERIC_NCR53C400 exists to enable or inhibit pseudo DMA
transfers when the driver is used with 53C400-compatible cards. Thanks to
Ondrej Zary's patches, PDMA now works which means it can be enabled
unconditionally.
Due to bad design, CONFIG_SCSI_GENERIC_NCR53C400 ties together unrelated
functionality as it sets both PSEUDO_DMA and BIOSPARAM macros. This patch
effectively enables PSEUDO_DMA and disables BIOSPARAM.
The defconfigs and the Kconfig default leave CONFIG_SCSI_GENERIC_NCR53C400
undefined. Red Hat 9 and CentOS 2.1 were the same. This leaves both
PSEUDO_DMA and BIOSPARAM disabled. The effect of this patch should be
better performance from enabling PSEUDO_DMA.
On the other hand, Debian 4 and SLES 10 had CONFIG_SCSI_GENERIC_NCR53C400
enabled, so both PSEUDO_DMA and BIOSPARAM were enabled. This patch might
affect configurations like this by disabling BIOSPARAM. My best guess is
that this could be a problem only in the vanishingly rare case that
1) the CHS values stored in the boot device partition table are wrong and
2) a 5380 card is in use (because PDMA on 53C400 used to be broken).

On 64 bit, struct error_info has 6 bytes of padding, which amounts to
over 4k of wasted space in the additional[] array. We could easily get
rid of that by instead using separate arrays for the codes and the
pointers. However, we can do even better than that and save an
additional 6 bytes per entry: In the table, just store the sizeof()
the corresponding string literal. The cumulative sum of these is then
the appropriate offset into additional_text, which is built from the
concatenation (with '\0's inbetween) of the strings.

The Kconfig help text used to say that CONFIG_SCSI_CONSTANTS=y costs
around 75 KB, but that was a little exaggerated. The actual number was
closer to 44K, and 36K with this patch.

A tablet PC booted into Windows may have its pen/touch hardware switched
into "Wacom mode" similar to what we do with explicitly-supported hardware.
Some devices appear to maintain this state across reboots, preventing their
use with the generic HID driver. This patch adds support for detecting the
presence of the mode switch feature report used by devices based on the G9
and G11 chips and has the HID codepath always attempt to reset the device
back to sending standard HID reports.

Commit 5ae6e89 introduced hid_data.inputmode with a comment that it
would have the value -1 if undefined, but then forgot to actually
perform the initialization. Although this doesn't appear to have
caused any problems in practice, it should still be remedied.

input_mt_get_slot_by_key() requires input_mt_sync_frame() to be called
at each frame. Do it when releasing the touches, or else we won't get
a proper slot number after mt_reset_resume().

Looks like recent changes in the Wacom driver made the Bamboo ONE crashes.
The tablet behaves as if it was a regular Bamboo device with pen, touch
and pad, but there is no physical pad connected to it.
The weird part is that the pad is still sending events and given that
there is no input node connected to it, we get anull pointer exception.

The lenovo_send_cmd_cptkbd function uses a stack variable to submit
commands via hid_hw_raw_request. Eventually this gets to the
usb_hcd_map_urb_for_dma function, which causes a warning to be thrown
if the CONFIG_DMA_API_DEBUG option is enabled.
Fix this by allocating a temporary buffer instead.

The usbhid driver has inconsistently duplicated code in its post-reset,
resume, and reset-resume pathways.
reset-resume doesn't check HID_STARTED before trying to
restart the I/O queues.
resume fails to clear the HID_SUSPENDED flag if HID_STARTED
isn't set.
resume calls usbhid_restart_queues() with usbhid->lock held
and the others call it without holding the lock.
The first item in particular causes a problem following a reset-resume
if the driver hasn't started up its I/O. URB submission fails because
usbhid->urbin is NULL, and this triggers an unending reset-retry loop.
This patch fixes the problem by creating a new subroutine,
hid_restart_io(), to carry out all the common activities. It also
adds some checks that were missing in the original code:
After a reset, there's no need to clear any halted endpoints.
After a resume, if a reset is pending there's no need to
restart any I/O until the reset is finished.
After a resume, if the interrupt-IN endpoint is halted there's
no need to submit the input URB until the halt has been
cleared.

The wiimote motion plus gyros use two scales to report fast and slow
rotation - below 440 deg/s uses 8192/440 units / deg/s, and above uses
8192/2000 units / deg/s.
Previously this driver attempted to linearize the two by scaling the fast
rate by 18 and the slow by 9, but this results in a scale of
8192*9/440 = ~167.564 for slow and 8192*18/2000 = 73.728 for fast.
Correct the fast motion scale factor so that both report ~167.564
units / deg/s

* Linux must be built with support of loop block devices. Thus it's
necessary to enable these Linux kernel options:
CONFIG_BLK_DEV

enabling loop block devices.
That among other things lead to additional 10 fatal signals
appearing during LTP run.

This is needed to get ethernet(stmmac) working in 4.6-rc2 on axs103.
4.5 needed this fix, but apprently stmmac has gained some fixes which
warrant reversal of this.

During mmaping of frame-buffer pages to user-space
fb_protect() is called to set proper page settings.
In case of ARC we need to mark pages that are mmaped to
user as uncached because of 2 reasons:
* Huge amount of data if passing through data cache will
thrash cache a lot making cache almost useless for other
less traffic hungry processes.
* Data written by user in FB will be immediately available for
hardware (such as PGU etc) without requirements to flush data
cache regularly.

Commit 5f8fc43217a0 ("PCI: Include pci/pcie/Kconfig directly from
pci/Kconfig") in linux-next changed drivers/pci/Kconfig to include
drivers/pci/pcie/Kconfig itself, so that architectures do not need
to source both files themselves. ARC just recently gained PCI support
through commit 6b3fb77998dd ("ARC: Add PCI support"), but this change
was based on the old behaviour of the Kconfig files. This makes
Kconfig now spit out the following warnings:
drivers/pci/pcie/Kconfig:61:warning: choice value used outside its choice group
drivers/pci/pcie/Kconfig:67:warning: choice value used outside its choice group
drivers/pci/pcie/Kconfig:74:warning: choice value used outside its choice group
This change updates the Kconfig file for ARC, dropping the now
unnecessary 'source' statement, which makes the warning disappear.

It was reported that a kernel with CONFIG_ARM_PATCH_IDIV=y stopped
booting when compiled with the upcoming gcc 6. Turns out that turning
a function address into a writable array is undefined and gcc 6 decided
it was OK to omit the store to the first word of the function while
still preserving the store to the second word.
Even though gcc 6 is now fixed to behave more coherently, it is a
mystery that gcc 4 and gcc 5 actually produce wanted code in the kernel.
And in fact the reduced test case to illustrate the issue does indeed
break with gcc < 6 as well.
In any case, let's guard the kernel against undefined compiler behavior
by hiding the nature of the array location as suggested by gcc
developers.

Masahiro Yamada reports that we can fail to set the FW bit in the
auxiliary control register, which enables broadcasting the cache
maintanence operations. This occurs because we only check that the
SMP/nAMP bit is set, rather than checking whether all the bits we
want to be set are set.
Rearrange the code to ensure that all desired bits are set, and only
update the register if we discover some required bits are not set.

Add support and PCI IDs for more Broxton host controllers
Other BXT IDs were added in v4.4 so cc'ing stable. This patch
is dependent on commit 163cbe31e516 ("mmc: sdhci-pci: Fix card
detect race for Intel BXT/APL") but that is already in stable
since v4.4.4.

Fix by adding a 'set_power' callback and restoring the default
behaviour prior to commit 918f4cbd4340 ("mmc: sdhci: restore
behavior when setting VDD via external regulator"). The desired
behaviour of that commit is gotten by having sdhci-pxav3 provide
its own set_power callback.
