This commit adds an option to set the device block size used to 4K.
By default Squashfs sets the device block size (sb_min_blocksize) to 1K
or the smallest block size supported by the block device (if larger).
This, because blocks are packed together and unaligned in Squashfs,
should reduce latency.
This, however, gives poor performance on MTD NAND devices where
the optimal I/O size is 4K (even though the devices can support
smaller block sizes).
Using a 4K device block size may also improve overall I/O
performance for some file access patterns (e.g. sequential
accesses of files in filesystem order) on all media.

The kernel code was using some <asm> headers that included a mix
of hardware-specific information (typically found in Tilera <arch>
headers) and structures, enums, and function declarations supporting
the disassembly function of the tile-desc.c sources.
This change refactors that code so that a hardware-specific, but
OS- and application-agnostic header, is created: <arch/opcode.h>.
This header is then exported to userspace along with the other
<arch> headers and can be used to build userspace code; in particular,
it is used by glibc as part of implementing the backtrace() function.
The new header, together with a header that specifically describes
the disassembly code (<asm/tile-desc.h> with _32 and _64 variants),
replaces the old <asm/opcode-tile*.h> and <asm/opcode_constants*.h>
headers.
As part of this change, we are also renaming the 32-bit constants
from TILE_xxx to TILEPRO_xxx to better reflect the fact that they
are specific to the TILEPro architecture, and not to TILE-Gx
and any successor "tile" architecture chips.

These headers are similar to the <asm> headers that describe kernel
APIs, but instead describe aspects of the actual hardware in an
OS- and application-independent manner. We need to include them in
the set of installed headers so that userspace tools (including glibc)
can build purely from the provided kernel headers.

An earlier Tilera compiler generated calls to an "__ll_mul"
function for long long multiplication. Our libgcc supported that
as an alias for the normal __muldi3 routine, so we made it available
to kernel modules as well. However, for a while now the compiler
has internally been generating only the standard __muldi3 symbol,
and the version we are giving back to the community does not have
the __ll_mul alias, so we are removing it from the kernel too.

<asm/sigcontext.h> is used by glibc's <bits/sigcontext.h> from <signal.h>,
which means that it can't clutter the namespace with random symbols
or #defines. However, we use <arch/abi.h> to get a suitable type to
hold a machine register.
This change makes <arch/abi.h> safe to use in this kind of context
if __need_int_reg_t is defined prior to including the file; in that
case, it only defines a few symbols that are safe in the ISO namespace
(prefixed with double underscores). <asm/sigcontext.h> then uses
the __uint_reg_t type instead of the normal uint_reg_t.

In this patch we are actually moving to the ORE.
(Object Raid Engine).
objio_state holds a pointer to an ore_io_state. Once
we have an ore_io_state at hand we can call the ore
for reading/writing. We register on the done path
to kick off the nfs io_done mechanism.
Again for Ease of reviewing the old code is "#if 0"
but is not removed so the diff command works better.
The old code will be removed in the next patch.
fs/exofs/Kconfig::ORE is modified to also be auto-included
if PNFS_OBJLAYOUT is set. Since we now depend on ORE.
(See comments in fs/exofs/Kconfig)

move to ore 01: ore_layout & ore_components
move to ore 02: move to ORE
move to ore 03: Remove old raid engine
This patch modifies the objio_lseg, layout-segment level
and devices and components arrays to use the ORE types.
Though it will be removed soon, also the raid engine
is modified to actually compile, possibly run, with
the new types. So it is the same old raid engine but
with some new ORE types.
For Ease of reviewing, some of the old code is
"#if 0" but is not removed so the diff command works
better. The old code will be removed in the 3rd patch.

This is part of moving objio_osd to use the ORE.
objlayout_io_state had two functions:
1. It was used in the error reporting mechanism at layout_return.
This function is kept intact.
(Later patch will rename objlayout_io_state => objlayout_io_res)
2. Carrier of rw io members into the objio_read/write_paglist API.
This is removed in this patch.
The {r,w}data received from NFS are passed directly to the
objio_{read,write}_paglist API. The io_engine is now allocating
it's own IO state as part of the read/write. The minimal
functionality that was part of the generic allocation is passed
to the io_engine.
So part of this patch is rename of:
ios->ol_state.foo => ios->foo
At objlayout_{read,write}_done an objlayout_io_state is passed that
denotes the result of the IO. (Hence the later name change).
If the IO is successful objlayout calls an objio_free_result() API
immediately (Which for objio_osd causes the release of the io_state).
If the IO ended in an error it is hanged onto until reported in
layout_return and is released later through the objio_free_result()
API. (All this is not new just renamed and cleaned)

objlayout driver was always returning PNFS_ATTEMPTED from it's
read/write_pagelist operations. Even on error. Fix that.
Start by establishing an error return API from io-engine, by
not returning ssize_t (length-or-error) but returning "int"
0=OK, 0>Error. And clean up all return types in io-engine.
Then if io-engine returned error return PNFS_NOT_ATTEMPTED
to generic layer. (With a dprint)

The EOF calculation was done on .read_pagelist(), cached
in objlayout_io_state->eof, and set in objlayout_read_done()
into nfs_read_data->res.eof.
So set it directly into nfs_read_data->res.eof and avoid
the extra member.

Current pnfs_layoutcommit_inode can not handle parallel layoutcommit.
And as Trond suggested , there is no need for client to optimize for
parallel layoutcommit. So add NFS_INO_LAYOUTCOMMITTING flag to
mark inflight layoutcommit and serialize lalyoutcommit with it.
Also mark_inode_dirty_sync if pnfs_layoutcommit_inode fails to issue
layoutcommit.

New function ("svc_uses_rpcbind") will be used to detect, that new service will
send portmapper register calls. For such services we will create rpcbind
clients and remove all stale portmap registrations.
Also, svc_rpcb_cleanup() will be set as sv_shutdown callback for such services
in case of this field wasn't initialized earlier. This will allow to destroy
rpcbind clients when no other users of them left.
Note: Currently, any creating service will be detected as portmap user.
Probably, this is wrong. But now it depends on program versions "vs_hidden"
flag.

The sunrpc layer keeps a cache of recently used credentials and
'unx_match' is used to find the credential which matches the current
process.
However unx_match allows a match when the cached credential has extra
groups at the end of uc_gids list which are not in the process group list.
So if a process with a list of (say) 4 group accesses a file and gains
access because of the last group in the list, then another process
with the same uid and gid, and a gid list being the first tree of the
gids of the original process tries to access the file, it will be
granted access even though it shouldn't as the wrong rpc credential
will be used.

RIP [<ffffffffa0168e8b>] qib_send_complete+0x3b/0x190 [ib_qib]
The RC error state flush logic in qib_make_rc_req() could return all
of the acked wqes and potentially have emptied the queue. It would
then unconditionally try return a flush completion via
qib_send_complete() for an invalid wqe, or worse a valid one that is
not queued. The panic results when the completion code tries to
maintain an MR reference count for a NULL MR.
This fix modifies logic to only send one completion per
qib_make_rc_req() call and changing the completion status from
IB_WC_SUCCESS to IB_WC_WR_FLUSH_ERR as the completions progress.
The outer loop will call as many times as necessary to flush the queue.

The num_free field of mthca_buddy has a type of array of unsigned int
while it was allocated as an array of pointers. On 64-bit platforms
this allocates twice more than required. Fix this by allocating the
correct size for the type.
This is the same bug just fixed in mlx4 by Eli Cohen <eli@mellanox.co.il>.

The current driver never does DMA unmapping on these buffers. Fix that
by adding DMA unmapping to the task cleanup callback, and DMA mapping to
the task init function (drop the headers_initialized micro-optimization).

The driver counted on the transactional nature of iSCSI login/text
flows and used the same buffer for both the request and the response.
We also went further and did DMA mapping only once, with
DMA_FROM_DEVICE, which violates the DMA mapping API. Fix that by
using different buffers, one for requests and one for responses, and
use the correct DMA mapping direction for each.
