We need to set cpsr before determining the spsr bank, as the bank
depends on the target exception level of the injection, not the
current mode of the vcpu. Normally this is one in the same (EL1),
but not when we manage to trap an EL0 fault. It still doesn't really
matter for the 64-bit EL0 case though, as vcpu_spsr() unconditionally
uses the EL1 bank for that. However the 32-bit EL0 case gets fun, as
that path will lead to the BUG() in vcpu_spsr32().
This patch fixes the assignment order and also modifies some white
space in order to better group pairs of lines that have strict order.

This patch adds compilation and link against irqchip.
Main motivation behind using irqchip code is to enable MSI
routing code. In the future irqchip routing may also be useful
when targeting multiple irqchips.
Routing standard callbacks now are implemented in vgic-irqfd:
- kvm_set_routing_entry
- kvm_set_irq
- kvm_set_msi
They only are supported with new_vgic code.
Both HAVE_KVM_IRQCHIP and HAVE_KVM_IRQ_ROUTING are defined.
KVM_CAP_IRQ_ROUTING is advertised and KVM_SET_GSI_ROUTING is allowed.
So from now on IRQCHIP routing is enabled and a routing table entry
must exist for irqfd injection to succeed for a given SPI. This patch
builds a default flat irqchip routing table (gsi=irqchip.pin) covering
all the VGIC SPI indexes. This routing table is overwritten by the
first first user-space call to KVM_SET_GSI_ROUTING ioctl.
MSI routing setup is not yet allowed.

set_pte_at(.) will set or unset the PTE_RDONLY hardware bit before
writing the entry to the table.
This can cause problems with the copy-on-write logic in hugetlb_cow:
*) hugetlb_cow(.) called to handle a write fault on read only pte,
*) Before the copy-on-write updates the new page table a call is
made to pte_same(huge_ptep_get(ptep), pte)), to check for a race,
*) Because set_pte_at(.) changed the pte, *ptep != pte, and the
hugetlb_cow(.) code erroneously assumes that it lost the race,
*) The new page is subsequently freed without being used.
On arm64 this problem only becomes apparent when we apply:
67961f9 mm/hugetlb: fix huge page reserve accounting for private
mappings
When one runs the libhugetlbfs test suite, there are allocation errors
and hugetlbfs pages become erroneously locked in memory as reserved.
(There is a high HugePages_Rsvd: count).
In this patch we introduce pte_same which ignores the PTE_RDONLY bit,
allowing for the libhugetlbfs test suite to pass as expected and
without leaking any reserved HugeTLB pages.

As reported by Zijun, the fdt_check_header() call in __fixmap_remap_fdt()
is not safe since it is not guaranteed that the FDT header is mapped
completely. Due to the minimum alignment of 8 bytes, the only fields we
can assume to be mapped are 'magic' and 'totalsize'.
Since the OF layer is in charge of validating the FDT image, and we are
only interested in making reasonably sure that the size field contains
a meaningful value, replace the fdt_check_header() call with an explicit
comparison of the magic field's value against the expected value.

AT_VECTOR_SIZE_ARCH should be defined with the maximum number of
NEW_AUX_ENT entries that ARCH_DLINFO can contain, but it wasn't defined
for arm64 at all even though ARCH_DLINFO will contain one NEW_AUX_ENT
for the VDSO address.
This shouldn't be a problem as AT_VECTOR_SIZE_BASE includes space for
AT_BASE_PLATFORM which arm64 doesn't use, but lets define it now and add
the comment above ARCH_DLINFO as found in several other architectures to
remind future modifiers of ARCH_DLINFO to keep AT_VECTOR_SIZE_ARCH up to
date.

The linker routines that we rely on to produce a relocatable PIE binary
treat it as a shared ELF object in some ways, i.e., it emits symbol based
R_AARCH64_ABS64 relocations into the final binary since doing so would be
appropriate when linking a shared library that is subject to symbol
preemption. (This means that an executable can override certain symbols
that are exported by a shared library it is linked with, and that the
shared library *must* update all its internal references as well, and point
them to the version provided by the executable.)
Symbol preemption does not occur for OS hosted PIE executables, let alone
for vmlinux, and so we would prefer to get rid of these symbol based
relocations. This would allow us to simplify the relocation routines, and
to strip the .dynsym, .dynstr and .hash sections from the binary. (Note
that these are tiny, and are placed in the .init segment, but they clutter
up the vmlinux binary.)
Note that these R_AARCH64_ABS64 relocations are only emitted for absolute
references to symbols defined in the linker script, all other relocatable
quantities are covered by anonymous R_AARCH64_RELATIVE relocations that
simply list the offsets to all 64-bit values in the binary that need to be
fixed up based on the offset between the link time and run time addresses.
Fortunately, GNU ld has a -Bsymbolic option, which is intended for shared
libraries to allow them to ignore symbol preemption, and unconditionally
bind all internal symbol references to its own definitions. So set it for
our PIE binary as well, and get rid of the asoociated sections and the
relocation code that processes them.

Due to the untyped KIMAGE_VADDR constant, the linker may not notice
that the __rela_offset and __dynsym_offset expressions are absolute
values (i.e., are not subject to relocation). This does not matter for
KASLR, but it does confuse kallsyms in relative mode, since it uses
the lowest non-absolute symbol address as the anchor point, and expects
all other symbol addresses to be within 4 GB of it.
Fix this by qualifying these expressions as ABSOLUTE() explicitly.

The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer. Thus the pointer can point to const data.
However the attributes do not have to be a bitfield. Instead unsigned
long will do fine:
1. This is just simpler. Both in terms of reading the code and setting
attributes. Instead of initializing local attributes on the stack
and passing pointer to it to dma_set_attr(), just set the bits.
2. It brings safeness and checking for const correctness because the
attributes are passed by value.

On systems with PCI_PROBE_ONLY set, we rely on BAR assignments from
firmware. Previously we did not insert those resources into the resource
tree, so we had to skip pci_enable_resources() because it fails if
resources are not in the resource tree.
Now that we *do* insert resources even when PCI_PROBE_ONLY is set, we no
longer need the ARM64-specific pcibios_enable_device(). Remove it so we
use the generic version.

The PCI_MSI symbol is used inconsistently throughout the tree, with some
drivers using 'select' and others using 'depends on', or using conditional

There is little reason to enable PCI support for a platform that uses MSI
but then leave MSI disabled at compile time.
Select PCI_MSI from irqchips that implement MSI, and make PCI host bridges
that use MSI on ARM depend on PCI_MSI_IRQ_DOMAIN.
For all three architectures that support PCI_MSI_IRQ_DOMAIN (ARM, ARM64,
X86), enable it by default whenever MSI is enabled.

Implement pci_acpi_scan_root() and other arch-specific calls so ARM64 can
use ACPI to setup and enumerate PCI buses.
Use memory-mapped configuration space information from either the ACPI
_CBA method or the MCFG table and the ECAM library and generic ECAM config
accessor ops.
Implement acpi_pci_bus_find_domain_nr() to retrieve the domain number from
the acpi_pci_root structure.
Implement pcibios_add_bus() and pcibios_remove_bus() to call
acpi_pci_add_bus() and acpi_pci_remove_bus() for ACPI slot management and
other configuration.

On ACPI systems, the PCI_Config OperationRegion allows AML to access PCI
configuration space. The ACPI CA AML interpreter uses performs config
space accesses with acpi_os_read_pci_configuration() and
acpi_os_write_pci_configuration(), which are OS-dependent functions
supplied by acpi/osl.c.
Implement the arch-specific raw_pci_read() and raw_pci_write() interfaces
used by acpi/osl.c for PCI_Config accesses.
N.B. PCI_Config accesses are not supported before PCI bus enumeration.

To enable PCI legacy IRQs on platforms booting with ACPI, arch code should
include ACPI-specific callbacks that parse and set-up the device IRQ
number, equivalent to the DT boot path. Owing to the current ACPI core scan
handlers implementation, ACPI PCI legacy IRQs bindings cannot be parsed at
device add time, since that would trigger ACPI scan handlers ordering
issues depending on how the ACPI tables are defined.
To solve this problem and consolidate FW PCI legacy IRQs parsing in one
single pcibios callback (pending final removal), this patch moves DT PCI
IRQ parsing to the pcibios_alloc_irq() callback (called by PCI core code at
driver probe time) and adds ACPI PCI legacy IRQs parsing to the same
callback too, so that FW PCI legacy IRQs parsing is confined in one single
arch callback that can be easily removed when code parsing PCI legacy IRQs
is consolidated and moved to core PCI code.

This patch allows to build the whole kernel with GCC plugins. It was ported from
grsecurity/PaX. The infrastructure supports building out-of-tree modules and
building in a separate directory. Cross-compilation is supported too.
Currently the x86, arm, arm64 and uml architectures enable plugins.
The directory of the gcc plugins is scripts/gcc-plugins. You can use a file or a directory
there. The plugins compile with these options:
* -fno-rtti: gcc is compiled with this option so the plugins must use it too
* -fno-exceptions: this is inherited from gcc too
* -fasynchronous-unwind-tables: this is inherited from gcc too
* -ggdb: it is useful for debugging a plugin (better backtrace on internal
errors)
* -Wno-narrowing: to suppress warnings from gcc headers (ipa-utils.h)
* -Wno-unused-variable: to suppress warnings from gcc headers (gcc_version
variable, plugin-version.h)
The infrastructure introduces a new Makefile target called gcc-plugins. It
supports all gcc versions from 4.5 to 6.0. The scripts/gcc-plugin.sh script
chooses the proper host compiler (gcc-4.7 can be built by either gcc or g++).
This script also checks the availability of the included headers in
scripts/gcc-plugins/gcc-common.h.
The gcc-common.h header contains frequently included headers for GCC plugins
and it has a compatibility layer for the supported gcc versions.
The gcc-generate-*-pass.h headers automatically generate the registration
structures for GIMPLE, SIMPLE_IPA, IPA and RTL passes.
Note that 'make clean' keeps the *.so files (only the distclean or mrproper
targets clean all) because they are needed for out-of-tree modules.

Introduce a new KVM device that represents an ARM Interrupt Translation
Service (ITS) controller. Since there can be multiple of this per guest,
we can't piggy back on the existing GICv3 distributor device, but create
a new type of KVM device.
On the KVM_CREATE_DEVICE ioctl we allocate and initialize the ITS data
structure and store the pointer in the kvm_device data.
Upon an explicit init ioctl from userland (after having setup the MMIO
address) we register the handlers with the kvm_io_bus framework.
Any reference to an ITS thus has to go via this interface.

KVM capabilities can be a per-VM property, though ARM/ARM64 currently
does not pass on the VM pointer to the architecture specific
capability handlers.
Add a "struct kvm*" parameter to those function to later allow proper
per-VM capability reporting.

So far, KVM was getting in the way of kexec on 32bit (and the arm64
kexec hackers couldn't be bothered to fix it on 32bit...).
With simpler page tables, tearing KVM down becomes very easy, so
let's just do it.

boot_pgd is useless and can be removed. We still keep it as
an element of the "extended idmap" thing.

We're in a position where we can now always have "merged" page
tables, where both the runtime mapping and the idmap coexist.
This results in some code being removed, but there is more to come.

As we move towards a selectable HYP VA range, it is obvious that
we don't want to test a variable to find out if we need to use
the bottom VA range, the top VA range, or use the address as is
(for VHE).
Instead, we can expand our current helper to generate the right
mask or nop with code patching. We default to using the top VA
space, with alternatives to switch to the bottom one or to nop
out the instructions.

As we need to indicate to the rest of the kernel which region of
the HYP VA space is safe to use, add a capability that will

HYP_PAGE_OFFSET is not massively useful. And the way we use it
in KERN_HYP_VA is inconsistent with the equivalent operation in
EL2, where we use a mask instead.
Let's replace the uses of HYP_PAGE_OFFSET with HYP_PAGE_OFFSET_MASK,
and get rid of the pointless macro.

__hyp_panic_string is passed via the HYP panic code to the panic
function, and is being "upgraded" to a kernel address, as it is
referenced by the HYP code (in a PC-relative way).
This is a bit silly, and we'd be better off obtaining the kernel
address and not mess with it at all. This patch implements this
with a tiny bit of asm glue, by forcing the string pointer to be
read from the literal pool.

I don't think any single piece of the KVM/ARM code ever generated
as much hatred as the GIC emulation.
It was written by someone who had zero experience in modeling
hardware (me), was riddled with design flaws, should have been
scrapped and rewritten from scratch long before having a remote
chance of reaching mainline, and yet we supported it for a good
three years. No need to mention the names of those who suffered,
the git log is singing their praises.
Thankfully, we now have a much more maintainable implementation,
and we can safely put the grumpy old GIC to rest.
Fellow hackers, please raise your glass in memory of the GIC:
The GIC is dead, long live the GIC!

Structures that can be generally written to don't have any requirement
to be executable (quite the opposite). This includes the kvm and vcpu
structures, as well as the stacks.
Let's change the default to incorporate the XN flag.

There should be no reason for mapping the HYP text read/write.
As such, let's have a new set of flags (PAGE_HYP_EXEC) that allows
execution, but makes the page as read-only, and update the two call
sites that deal with mapping code.

In order to be able to use C code in HYP, we're now mapping the kernel's
rodata in HYP. It works absolutely fine, except that we're mapping it RWX,
which is not what it should be.
Add a new HYP_PAGE_RO protection, and pass it as the protection flags
when mapping the rodata section.
