The Linux implementation of PCI error recovery for s390 was based on the
understanding that firmware error recovery is a two step process with an
optional initial error event to indicate the cause of the error if known
followed by either error event 0x3A (Success) or 0x3B (Failure) to
indicate whether firmware was able to recover. While this has been the
case in testing and the error cases seen in the wild it turns out this
is not correct. Instead firmware only generates 0x3A for some error and
service scenarios and expects the OS to perform recovery for all PCI
events codes except for those indicating permanent error (0x3B, 0x40)
and those indicating errors on the function measurement block (0x2A,
0x2B, 0x2C). Align Linux behavior with these expectations.

no_llseek had been defined to NULL two years ago, in commit 868941b14441
("fs: remove no_llseek")
To quote that commit,
At -rc1 we'll need do a mechanical removal of no_llseek -
git grep -l -w no_llseek | grep -v porting.rst | while read i; do
sed -i '/\<no_llseek\>/d' $i
done
would do it.
Unfortunately, that hadn't been done. Linus, could you do that now, so
that we could finally put that thing to rest? All instances are of the
form
.llseek = no_llseek,
so it's obviously safe.

Pull pci updates from Bjorn Helgaas:
"Enumeration:
- Wait for device readiness after reset by polling Vendor ID and
looking for Configuration RRS instead of polling the Command
register and looking for non-error completions, to avoid hardware
retries done for RRS on non-Vendor ID reads (Bjorn Helgaas)
- Rename CRS Completion Status to RRS ('Request Retry Status') to
match PCIe r6.0 spec usage (Bjorn Helgaas)
- Clear LBMS bit after a manual link retrain so we don't try to
retrain a link when there's no downstream device anymore (Maciej W.
Rozycki)
- Revert to the original link speed after retraining fails instead of
leaving it restricted to 2.5GT/s, so a future device has a chance
to use higher speeds (Maciej W. Rozycki)
- Wait for each level of downstream bus, not just the first, to
become accessible before restoring devices on that bus (Ilpo
Järvinen)
- Add ARCH_PCI_DEV_GROUPS so s390 can add its own attribute_groups

Bjorn suggests using pdev->dev.groups for attribute_groups constructed on
PCI device enumeration:
"Is it feasible to build an attribute group in pci_doe_init() and
add it to dev->groups so device_add() will automatically add them?"
https://lore.kernel.org/r/20231019165829.GA1381099@bhelgaas
Unfortunately on s390, pcibios_device_add() usurps pdev->dev.groups for
arch-specific attribute_groups, preventing its use for anything else.
Introduce an ARCH_PCI_DEV_GROUPS macro which arches can define in
<asm/pci.h>. The macro is visible in drivers/pci/pci-sysfs.c through the
inclusion of <linux/pci.h>, which in turn includes <asm/pci.h>.
On s390, define the macro to the three attribute_groups previously assigned
to pdev->dev.groups. Thereby pdev->dev.groups is made available for use by
the PCI core.
As a side effect, arch/s390/pci/pci_sysfs.c no longer needs to be compiled
into the kernel if CONFIG_SYSFS=n.

On a PCI adapter that provides up to 8 MSI interrupt sources the s390
implementation of PCI interrupts rejected to accommodate them, although
the underlying hardware is able to support that.
For MSI-X it is sufficient to allocate a single irq_desc per msi_desc,
but for MSI multiple irq descriptors are attached to and controlled by
a single msi descriptor. Add the appropriate loops to maintain multiple
irq descriptors and tie/untie them to/from the appropriate AIBV bit, if
a device driver allocates more than 1 MSI interrupt.
Common PCI code passes on requests to allocate a number of interrupt
vectors based on the device drivers' demand and the PCI functions'
capabilities. However, the root-complex of s390 systems support just a
limited number of interrupt vectors per PCI function.
Produce a kernel log message to inform about any architecture-specific
capping that might be done.
With this change, we had a PCI adapter successfully raising
interrupts to its device driver via all 8 sources.

Pull s390 updates from Vasily Gorbik:
- Remove restrictions on PAI NNPA and crypto counters, enabling
concurrent per-task and system-wide sampling and counting events
- Switch to GENERIC_CPU_DEVICES by setting up the CPU present mask in
the architecture code and letting the generic code handle CPU
bring-up
- Add support for the diag204 busy indication facility to prevent
undesirable blocking during hypervisor logical CPU utilization
queries. Implement results caching

unnecessary warning, preventing buffer release in I/O during
failures, and adding timeout handling for Store Data requests to
address potential firmware issues
- Provide optimized __arch_hweight*() implementations
- Remove the unnecessary CPU KOBJ_CHANGE uevents generated during
topology updates, as they are unused and also not present on other
architectures

In commit 4e4dc65ab578 ("s390/pci: use phys_to_virt() for AIBVs/DIBVs")
the setting of dibv_addr was missed when adding virt_to_phys(). This
only affects systems with directed interrupt delivery enabled which are
not generally available.

arch provided inline/macro functions instead of weak symbols.
S390 has an implementation that simply calls another memcpy
function. Inline this so the callers don't have to do two jumps.

git://git.kernel.org/pub/scm/linux/kernel/git/s390/linux
Pull s390 updates from Heiko Carstens:
- Various virtual vs physical address usage fixes
- Fix error handling in Processor Activity Instrumentation device
driver, and export number of counters with a sysfs file
- Allow for multiple events when Processor Activity Instrumentation
counters are monitored in system wide sampling
- Change multiplier and shift values of the Time-of-Day clock source to
improve steering precision
- Remove a couple of unneeded GFP_DMA flags from allocations
- Disable mmap alignment if randomize_va_space is also disabled, to
avoid a too small heap
- Various changes to allow s390 to be compiled with LLVM=1, since
ld.lld and llvm-objcopy will have proper s390 support witch clang 19
- Add __uninitialized macro to Compiler Attributes. This is helpful
with s390's FPU code where some users have up to 520 byte stack
frames. Clearing such stack frames (if INIT_STACK_ALL_PATTERN or
INIT_STACK_ALL_ZERO is enabled) before they are used contradicts the
intention (performance improvement) of such code sections.
- Convert switch_to() to an out-of-line function, and use the generic
switch_to header file
- Replace the usage of s390's debug feature with pr_debug() calls
within the zcrypt device driver
- Improve hotplug support of the Adjunct Processor device driver
- Improve retry handling in the zcrypt device driver
- Various changes to the in-kernel FPU code:
- Make in-kernel FPU sections preemptible
- Convert various larger inline assemblies and assembler files to
C, mainly by using singe instruction inline assemblies. This
increases readability, but also allows makes it easier to add
proper instrumentation hooks
- Cleanup of the header files
- Provide fast variants of csum_partial() and
csum_partial_copy_nocheck() based on vector instructions
- Introduce and use a lock to synchronize accesses to zpci device data
structures to avoid inconsistent states caused by concurrent accesses
- Compile the kernel without -fPIE. This addresses the following
problems if the kernel is compiled with -fPIE:
- It uses dynamic symbols (.dynsym), for which the linker refuses
to allow more than 64k sections. This can break features which
use '-ffunction-sections' and '-fdata-sections', including
kpatch-build and function granular KASLR
- It unnecessarily uses GOT relocations, adding an extra layer of
indirection for many memory accesses
- Fix shared_cpu_list for CPU private L2 caches, which incorrectly were
reported as globally shared

There's a number of tasks that need the state of a zpci device
to be stable. Other tasks need to be synchronized as they change the state.
State changes could be generated by the system as availability or error
events, or be requested by the user through manipulations in sysfs.
Some other actions accessible through sysfs - like device resets - need the
state to be stable.
Unsynchronized state handling could lead to unusable devices. This has
been observed in cases of concurrent state changes through systemd udev
rules and DPM boot control. Some breakage can be provoked by artificial
tests, e.g. through repetitively injecting "recover" on a PCI function
through sysfs while running a "hotplug remove/add" in a loop through a
PCI slot's "power" attribute in sysfs. After a few iterations this could
result in a kernel oops.
So introduce a new mutex "state_lock" to guard the state property of the
struct zpci_dev. Acquire this lock in all task that modify the state:
- hotplug add and remove, through the PCI hotplug slot entry,
- avaiability events, as reported by the platform,
- error events, as reported by the platform,
- during device resets, explicit through sysfs requests or
implict through the common PCI layer.
Break out an inner _do_recover() routine out of recover_store() to
separte the necessary synchronizations from the actual manipulations of
the zpci_dev required for the reset.
With the following changes I was able to run the inject loops for hours
without hitting an error.

The zpci_get_max_write_size() helper is used to determine the maximum
size a PCI store or load can use at a given __iomem address.
For the PCI block store the following restrictions apply:
1. The dst + len must not cross a 4K boundary in the (pseudo-)MMIO space
2. len must not exceed ZPCI_MAX_WRITE_SIZE
3. len must be a multiple of 8 bytes
4. The src address must be double word (8 byte) aligned
5. The dst address must be double word (8 byte) aligned
Otherwise only a normal PCI store which takes its src value from
a register can be used. For these PCI store restriction 1 still applies.
Similarly 1 also applies to PCI loads.
It turns out zpci_max_write_size() instead implements stricter
conditions which prevents PCI block stores from being used where they
can and should be used. In particular instead of conditions 4 and 5 it
wrongly enforces both dst and src to be size aligned. This indirectly
covers condition 1 but also prevents many legal PCI block stores.
On top of the functional shortcomings the zpci_get_max_write_size() is
misnamed as it is used for both read and write size calculations. Rename
it to zpci_get_max_io_size() and implement the listed conditions
explicitly.

Together with enabling the Function Measurement Block
zpci_fmb_enable_device() also resets the software counters. This allows
to use "echo 0 > /sys/kernel/debug/pci/<dev>/statistics" followed by
echo "1 > /../statistics" to reset all counters. In commit c76c067e488c
("s390/pci: Use dma-iommu layer") this use of the now obsolete counters
in struct zpci_device was missed as was their removal. Fix this by
resetting the new counters and removing the old ones.

While s390 already has a standard IOMMU driver and previous changes have
added I/O TLB flushing operations this driver is currently only used for
user-space PCI access such as vfio-pci. For the DMA API s390 instead
utilizes its own implementation in arch/s390/pci/pci_dma.c which drives
the same hardware and shares some code but requires a complex and
fragile hand over between DMA API and IOMMU API use of a device and
despite code sharing still leads to significant duplication and
maintenance effort. Let's utilize the common code DMAP API
implementation from drivers/iommu/dma-iommu.c instead allowing us to
get rid of arch/s390/pci/pci_dma.c.

Since the fixed commits both zdev->iommu_bitmap and zdev->lazy_bitmap
are allocated as vzalloc(zdev->iommu_pages / 8). The problem is that
zdev->iommu_bitmap is a pointer to unsigned long but the above only
yields an allocation that is a multiple of sizeof(unsigned long) which
is 8 on s390x if the number of IOMMU pages is a multiple of 64.
This in turn is the case only if the effective IOMMU aperture is
a multiple of 64 * 4K = 256K. This is usually the case and so didn't
cause visible issues since both the virt_to_phys(high_memory) reduced
limit and hardware limits use nice numbers.
Under KVM, and in particular with QEMU limiting the IOMMU aperture to
the vfio DMA limit (default 65535), it is possible for the reported
aperture not to be a multiple of 256K however. In this case we end up
with an iommu_bitmap whose allocation is not a multiple of
8 causing bitmap operations to access it out of bounds.
Sadly we can't just fix this in the obvious way and use bitmap_zalloc()
because for large RAM systems (tested on 8 TiB) the zdev->iommu_bitmap
grows too large for kmalloc(). So add our own bitmap_vzalloc() wrapper.
This might be a candidate for common code, but this area of code will
be replaced by the upcoming conversion to use the common code DMA API on
s390 so just add a local routine.

By taking GENERIC_IOREMAP method, the generic generic_ioremap_prot(),
generic_iounmap(), and their generic wrapper ioremap_prot(), ioremap() and
iounmap() are all visible and available to arch. Arch needs to provide
wrapper functions to override the generic versions if there's arch
specific handling in its ioremap_prot(), ioremap() or iounmap(). This
change will simplify implementation by removing duplicated code with
generic_ioremap_prot() and generic_iounmap(), and has the equivalent
functioality as before.
Here, add wrapper functions ioremap_prot() and iounmap() for s390's
special operation when ioremap() and iounmap().
And also replace including <asm-generic/io.h> with <asm/io.h> in
arch/s390/kernel/perf_cpum_sf.c, otherwise building error will be seen
because macro defined in <asm/io.h> can't be seen in perf_cpum_sf.c.

Heiko Carstens says:
===================
There are a couple of oddities within the s390 uaccess library
functions. Therefore cleanup the whole uaccess.c file.
There is no functional change, only improved readability. The output
of "objdump -Dr" was always compared before/after each patch to make
sure that the generated object file is identical, if that could be
expected. Therefore the series also includes more patches than really
required to cleanup the code.
Furthermore the kunit usercopy tests also still pass.

On s390 PCI functions may be hotplugged individually even when they
belong to a multi-function device. In particular on an SR-IOV device VFs
may be removed and later re-added.
In commit a50297cf8235 ("s390/pci: separate zbus creation from
scanning") it was missed however that struct pci_bus and struct
zpci_bus's resource list retained a reference to the PCI functions MMIO
resources even though those resources are released and freed on
hot-unplug. These stale resources may subsequently be claimed when the
PCI function re-appears resulting in use-after-free.
One idea of fixing this use-after-free in s390 specific code that was
investigated was to simply keep resources around from the moment a PCI
function first appeared until the whole virtual PCI bus created for
a multi-function device disappears. The problem with this however is
that due to the requirement of artificial MMIO addreesses (address
cookies) extra logic is then needed to keep the address cookies
compatible on re-plug. At the same time the MMIO resources semantically
belong to the PCI function so tying their lifecycle to the function
seems more logical.
Instead a simpler approach is to remove the resources of an individually
hot-unplugged PCI function from the PCI bus's resource list while
keeping the resources of other PCI functions on the PCI bus untouched.
This is done by introducing pci_bus_remove_resource() to remove an
individual resource. Similarly the resource also needs to be removed
from the struct zpci_bus's resource list. It turns out however, that
there is really no need to add the MMIO resources to the struct
zpci_bus's resource list at all and instead we can simply use the
zpci_bar_struct's resource pointer directly.

Prior to commit 960ac3626487 ("s390/pci: allow zPCI zbus without
a function zero") enabling and scanning a PCI function had to
potentially be postponed until the function with devfn zero on that bus
was plugged. While the commit removed the waiting itself extra code to
scan all functions on the PCI bus once function zero appeared was
missed. Remove that code and the outdated comments about waiting for
function zero.

I/O translation tables on s390 use 8 byte page table entries and tables
which are allocated lazily but only freed when the entire I/O
translation table is torn down. Also each IOVA can at any time only
translate to one physical address Furthermore I/O table accesses by the
IOMMU hardware are cache coherent. With a bit of care we can thus use
atomic updates to manipulate the translation table without having to use
a global lock at all. This is done analogous to the existing I/O
translation table handling code used on Intel and AMD x86 systems.

The s390_domain->devices list is only added to when new devices are
attached but is iterated through in read-only fashion for every mapping
operation as well as for I/O TLB flushes and thus in performance
critical code causing contention on the s390_domain->list_lock.
Fortunately such a read-mostly linked list is a standard use case for
RCU. This change closely follows the example fpr RCU protected list
given in Documentation/RCU/listRCU.rst.

If a zPCI device is in the error state while switching IOMMU domains
zpci_register_ioat() will fail and we would end up with the device not
attached to any domain. In this state since zdev->dma_table == NULL
a reset via zpci_hot_reset_device() would wrongfully re-initialize the
device for DMA API usage using zpci_dma_init_device(). As automatic
recovery is currently disabled while attached to an IOMMU domain this
only affects slot resets triggered through other means but will affect
automatic recovery once we switch to using dma-iommu.
Additionally with that switch common code expects attaching to the
default domain to always work so zpci_register_ioat() should only fail
if there is no chance to recover anyway, e.g. if the device has been
unplugged.
Improve the robustness of attach by specifically looking at the status
returned by zpci_mod_fc() to determine if the device is unavailable and
in this case simply ignore the error. Once the device is reset
zpci_hot_reset_device() will then correctly set the domain's DMA
translation tables.

Pull s390 updates from Alexander Gordeev:
- Factor out handle_write() function and simplify 3215 console write
operation
- When 3170 terminal emulator is connected to the 3215 console driver
the boot time could be very long due to limited buffer space or
missing operator input. Add con3215_drop command line parameter and
con3215_drop sysfs attribute file to instruct the kernel drop console
data when such conditions are met
- Fix white space errors in 3215 console driver
- Move enum paiext_mode definition to a header file and rename it to
paievt_mode to indicate this is now used for several events. Rename
PAI_MODE_COUNTER to PAI_MODE_COUNTING to make consistent with
PAI_MODE_SAMPLING
- Simplify the logic of PMU pai_crypto mapped buffer reference counter
and make it consistent with PMU pai_ext
- Rename PMU pai_crypto mapped buffer structure member users to
active_events to make it consistent with PMU pai_ext
- Enable HUGETLB_PAGE_OPTIMIZE_VMEMMAP configuration option. This
results in saving of 12K per 1M hugetlb page (~1.2%) and 32764K per
2G hugetlb page (~1.6%)
- Use generic serial.h, bugs.h, shmparam.h and vga.h header files and
scrap s390-specific versions
- The generic percpu setup code does not expect the s390-like
implementation and emits a warning. To get rid of that warning and
provide sane CPU-to-node and CPU-to-CPU distance mappings implementat
a minimal version of setup_per_cpu_areas()
- Use kstrtobool() instead of strtobool() for re-IPL sysfs device
attributes
- Avoid unnecessary lookup of a pointer to MSI descriptor when setting
IRQ affinity for a PCI device
- Get rid of "an incompatible function type cast" warning by changing
debug_sprintf_format_fn() function prototype so it matches the
debug_format_proc_t function type
- Remove unused info_blk_hdr__pcpus() and get_page_state() functions
- Get rid of clang "unused unused insn cache ops function" warning by
moving s390_insn definition to a private header
- Get rid of clang "unused function" warning by making function
raw3270_state_final() only available if CONFIG_TN3270_CONSOLE is
enabled
- Use kstrobool() to parse sclp_con_drop parameter to make it identical
to the con3215_drop parameter and allow passing values like "yes" and
"true"
- Use sysfs_emit() for all SCLP sysfs show functions, which is the
current standard way to generate output strings
- Make SCLP con_drop sysfs attribute also writable and allow to change
its value during runtime. This makes SCLP console drop handling
consistent with the 3215 device driver
- Virtual and physical addresses are indentical on s390. However, there
is still a confusion when pointers are directly casted to physical
addresses or vice versa. Use correct address converters
virt_to_phys() and phys_to_virt() for s390 channel IO drivers
- Support for power managemant has been removed from s390 since quite
some time. Remove unused power managemant code from the appldata
device driver
- Allow memory tools like KASAN see memory accesses from the checksum
code. Switch to GENERIC_CSUM if KASAN is enabled, just like x86 does
- Add support of ECKD DASDs disks so it could be used as boot and dump
devices
- Follow checkpatch recommendations and use octal values instead of
S_IRUGO and S_IWUSR for dump device attributes in sysfs
- Changes to vx-insn.h do not cause a recompile of C files that use
asm(".include \"asm/vx-insn.h\"\n") magic to access vector
instruction macros from inline assemblies. Add wrapper include header
file to avoid this problem
- Use vector instruction macros instead of byte patterns to increase
register validation routine readability
- The current machine check register validation handling does not take
into account various scenarios and might lead to killing a wrong user
process or potentially ignore corrupted FPU registers. Simplify logic
of the machine check handler and stop the whole machine if the
previous context was kerenel mode. If the previous context was user
mode, kill the current task
- Introduce sclp_emergency_printk() function which can be used to emit
a message in emergency cases. It is supposed to be used in cases
where regular console device drivers may not work anymore, e.g.
unrecoverable machine checks
Keep the early Service-Call Control Block so it can also be used
after initdata has been freed to allow sclp_emergency_printk()
implementation
- In case a system will be stopped because of an unrecoverable machine
check error print the machine check interruption code to give a hint
of what went wrong
- Move storage error checking from the assembly entry code to C in
order to simplify machine check handling. Enter the handler with DAT
turned on, which simplifies the entry code even more
- The machine check extended save areas are allocated using a private
"nmi_save_areas" slab cache which guarantees a required power-of-two
alignment. Get rid of that cache in favour of kmalloc()

We use high_memory as a measure for amount of memory available in
determining the required minimum size of our IOVA space with the
assumption that one rarely maps more than the available memory for DMA.
In special cases like mapping significant amounts of memory more than
once this can still be tuned with the s390_iommu_apterture kernel
parameter. In this use case high_memory is treated as a physical
address. As high_memory is a virtual address however this means we need
to convert it using virt_to_phys() before use
Note that at the moment physical and virtual addresses are identical so
this mismatch does not currently cause trouble.
