Most arches have an asm/gpio.h that merely includes linux/gpio.h. The
others select ARCH_HAVE_CUSTOM_GPIO_H, and when that's selected,
linux/gpio.h includes asm/gpio.h.
Therefore, code should include linux/gpio.h instead of including asm/gpio.h
directly.
Remove includes of asm/gpio.h, adding an include of linux/gpio.h when
necessary.
This is a follow-on to 7563bbf89d06 ("gpiolib/arches: Centralise
bolierplate asm/gpio.h").

This makes the driver use the data pointer added to the gpio_chip
to store a pointer to the state container instead of relying on
container_of().

These new helpers simplify implementing multi-driver modules and
properly handle failure to register one driver by unregistering all
previously registered drivers.

Since d621e8bae5ac9c67 (Create of_mm_gpiochip_remove), there is a
counterpart for of_mm_gpiochip_add.
This patch implements the remove function of the driver making use of
it.

A pending cleanup will mean that module.h won't be implicitly
everywhere anymore. Make sure the modular drivers in gpio
are actually calling out for <module.h> explicitly in advance.

GPIO drivers are getting consolidated into drivers/gpio. While at it,
change the driver name to mpc5200-gpio* to avoid collisions.
