alloc_pages_exact_node() was introduced in commit 6484eb3e2a81 ("page
allocator: do not check NUMA node ID when the caller knows the node is
valid") as an optimized variant of alloc_pages_node(), that doesn't
fallback to current node for nid == NUMA_NO_NODE. Unfortunately the
name of the function can easily suggest that the allocation is
restricted to the given node and fails otherwise. In truth, the node is
only preferred, unless __GFP_THISNODE is passed among the gfp flags.
The misleading name has lead to mistakes in the past, see for example
commits 5265047ac301 ("mm, thp: really limit transparent hugepage
allocation to local node") and b360edb43f8e ("mm, mempolicy:
migrate_to_node should only migrate to node").
Another issue with the name is that there's a family of
alloc_pages_exact*() functions where 'exact' means exact size (instead
of page order), which leads to more confusion.
To prevent further mistakes, this patch effectively renames
alloc_pages_exact_node() to __alloc_pages_node() to better convey that
it's an optimized variant of alloc_pages_node() not intended for general
usage. Both functions get described in comments.
It has been also considered to really provide a convenience function for
allocations restricted to a node, but the major opinion seems to be that
__GFP_THISNODE already provides that functionality and we shouldn't
duplicate the API needlessly. The number of users would be small
anyway.
Existing callers of alloc_pages_exact_node() are simply converted to
call __alloc_pages_node(), with the exception of sba_alloc_coherent()
which open-codes the check for NUMA_NO_NODE, so it is converted to use
alloc_pages_node() instead. This means it no longer performs some
VM_BUG_ON checks, and since the current check for nid in
alloc_pages_node() uses a 'nid < 0' comparison (which includes
NUMA_NO_NODE), it may hide wrong values which would be previously
exposed.
Both differences will be rectified by the next patch.
To sum up, this patch makes no functional changes, except temporarily
hiding potentially buggy callers. Restricting the checks in
alloc_pages_node() is left for the next patch which can in turn expose
more existing buggy callers.

GFP_THISNODE is for callers that implement their own clever fallback to
remote nodes. It restricts the allocation to the specified node and
does not invoke reclaim, assuming that the caller will take care of it
when the fallback fails, e.g. through a subsequent allocation request
without GFP_THISNODE set.
However, many current GFP_THISNODE users only want the node exclusive
aspect of the flag, without actually implementing their own fallback or
triggering reclaim if necessary. This results in things like page
migration failing prematurely even when there is easily reclaimable
memory available, unless kswapd happens to be running already or a
concurrent allocation attempt triggers the necessary reclaim.
Convert all callsites that don't implement their own fallback strategy
to __GFP_THISNODE. This restricts the allocation a single node too, but
at the same time allows the allocator to enter the slowpath, wake
kswapd, and invoke direct reclaim if necessary, to make the allocation
happen when memory is full.

interruptible_sleep_on_timeout is deprecated and going away soon.
The use in the sgi-xp driver leaves me puzzled, so I'd prefer not
to touch it. This patch replaces it with an open-coded prepare_to_wait
and finish_wait pair, which should be completely equivalent, so it
doesn't fix an existing race, but lets us get away with removing
the function so we can not get any new users.
In order to remove the typical sleep_on race, one would have to
replace the call with wait_event_interruptible_timeout and add
a condition to wait for. The fact that there is a one-jiffy timeout
suggests that we don't actually expect to get woken up properly
and the caller just uses this as a short sleeping function
if it doesn't wake up properly.

We found a user code which was raising a divide-by-zero trap. That trap
would lead to XPC connections between system-partitions being torn down
due to the die_chain notifier callouts it received.
This also revealed a different issue where multiple callers into
xpc_die_deactivate() would all attempt to do the disconnect in parallel
which would sometimes lock up but often overwhelm the console on very
large machines as each would print at least one line of output at the
end of the deactivate.
I reviewed all the users of the die_chain notifier and changed the code
to ignore the notifier callouts for reasons which will not actually lead
to a system to continue on to call die().

resources
On many of our larger systems, CPU 0 has had all of its IRQ resources
consumed before XPC loads. Worst cases on machines with multiple 10
GigE cards and multiple IB cards have depleted the entire first socket
of IRQs.
This patch makes selecting the node upon which IRQs are allocated (as
well as all the other GRU Message Queue structures) specifiable as a
module load param and has a default behavior of searching all nodes/cpus
for an available resources.

The code here has a nested spin_lock_irqsave(). It's not needed since
IRQs are already disabled and it causes a problem because it means that
IRQs won't be enabled again at the end. The second call to
spin_lock_irqsave() will overwrite the value of irq_flags and we can't
restore the proper settings.

UV hardware defines 256 memory protection regions versus the baseline 64
with increasing size for the SN2 ia64. This was overlooked when XPC was
modified to accomodate both UV and SN2.
Without this patch, a user could reconfigure their existing system and
suddenly disable cross-partition communications with no indication of what
has gone wrong. It also prevents larger configurations from using
cross-partition communication.

partition structures have been torn down
Under some workloads, some channel messages have been observed being
delayed on the sending side past the point where the receiving side has
been able to tear down its partition structures.
This condition is already detected in xpc_handle_activate_IRQ_uv(), but
that information is not given to xpc_handle_activate_mq_msg_uv(). As a
result, xpc_handle_activate_mq_msg_uv() assumes the structures still exist
and references them, causing a NULL-pointer deref.

implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.
http://userweb.kernel.org/~tj/misc/slabh-sweep.py
The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

The patch includes a macro to map the old for_each_bit() onto the new

This was a difficult bug to trip. XPC was in the middle of sending an
acknowledgement for a received message.
In xpc_received_payload_uv():
.
ret = xpc_send_gru_msg(ch->sn.uv.cached_notify_gru_mq_desc, msg,
sizeof(struct xpc_notify_mq_msghdr_uv));
if (ret != xpSuccess)
XPC_DEACTIVATE_PARTITION(&xpc_partitions[ch->partid], ret);
msg->hdr.msg_slot_number += ch->remote_nentries;
at the point in xpc_send_gru_msg() where the hardware has dispatched the
acknowledgement, the remote side is able to reuse the message structure
and send a message with a different slot number. This problem is made
worse by interrupts.
The adjustment of msg_slot_number and the BUG_ON in
xpc_handle_notify_mq_msg_uv() which verifies the msg_slot_number is
consistent are only used for debug purposes. Since a fix for this that
preserves the debug functionality would either have to infringe upon the
payload or allocate another structure just for debug, I decided to remove
it entirely.

The UV BIOS has moved the location of some of their pointers to the
"partition reserved page" from memory into a uv hub MMR. The GRU does not
support bcopy operations from MMR space so we need to special case the MMR
addresses using VLOAD operations.
Additionally, the BIOS call for registering a message queue watchlist has
removed the 'blade' value and eliminated the structure that was being
passed in. This is also reflected in this patch.

For consistency drop & in front of every proc_handler. Explicity
taking the address is unnecessary and it prevents optimizations
like stubbing the proc_handlers to NULL.

This patch fixes handling of uv hub irq affinity. IRQs with ALL or
NODE affinity can be routed to cpus other than their originally
assigned cpu. Those with CPU affinity cannot be rerouted.

Commit 5fd29d6ccbc98884569d6f3105aeca70858b3e0f ("printk: clean up
handling of log-levels and newlines") changed printk semantics. printk
lines with multiple KERN_<level> prefixes are no longer emitted as
before the patch.
<level> is now included in the output on each additional use.
Remove all uses of multiple KERN_<level>s in formats.

valid
Callers of alloc_pages_node() can optionally specify -1 as a node to mean
"allocate from the current node". However, a number of the callers in
fast paths know for a fact their node is valid. To avoid a comparison and
branch, this patch adds alloc_pages_exact_node() that only checks the nid
with VM_BUG_ON(). Callers that know their node is valid are then
converted.

The heartbeat timeout functionality in sgi-xpc is currently not trained to
the connection time. If a connection is made and the code is in the last
polling window prior to doing a timeout, the next polling window will see
the heartbeat as unchanged and initiate a no-heartbeat disconnect.

A missing type cast results in writing way beyond the end of a kzalloc()'d
memory segment resulting in slab corruption. But it seems like the better
solution is to define ->recv_msg_slots as a 'void *' rather than a
'struct xpc_notify_mq_msg_uv *' and add the type cast.
