While shutting down loop controller, we first quiesce the admin/IO queue,
delete the admin/IO tag-set and then at last destroy the admin/IO queue.
However it's quite possible that during the window between quiescing and
destroying of the admin/IO queue, some admin/IO request might sneak in
and if that happens then we could potentially encounter a hung task
because shutdown operation can't forward progress until any pending I/O
is flushed off.
This commit helps ensure that before destroying the admin/IO queue, we
unquiesce the admin/IO queue so that any outstanding requests, which are
added after the admin/IO queue is quiesced, are now flushed to its
completion.

Combining both creates an ambiguous cleanup scenario for the caller if
an error is returned: does the device reference need to be dropped or
did the error occur before the device was initialized? If an error
occurs after the device is added, then the existing cleanup routines
will leak memory.
Furthermore, the nvme core is taking it upon itself to free the device's
kobj name under certain conditions rather than go through the core
device API. We shouldn't be peaking into these implementation details.
Split the device initialization from the addition to make it easier to
know the error handling actions, fix the existing memory leaks, and stop
the device layering violations.

All nvme transports should be using the same flags for their tagsets,
with the exception for the blocking flag that should only be set for
transports that can block in ->queue_rq.
Add a NVME_F_BLOCKING flag to nvme_ctrl_ops to control the blocking
behavior and lift setting the flags into nvme_alloc_{admin,io}_tag_set.

Nothing about the TCG Opal support is PCIe transport specific, so move it
to the core code. For this nvme_init_ctrl_finish grows a new
was_suspended argument that allows the transport driver to tell the OPAL
code if the controller came out of a suspend cycle.

Set the queue dying flag and call blk_mq_exit_queue from del_gendisk for
all disks that do not have separately allocated queues, and thus remove
the need to call blk_cleanup_queue for them.
Rename blk_cleanup_disk to blk_mq_destroy_queue to make it clear that
this function is intended only for separately allocated blk-mq queues.
This saves an extra queue freeze for devices without a separately
allocated queue.

Any attempt to flush kernel-global WQs has possibility of deadlock
so we should simply stop using them, instead introduce nvmet_wq
which is the generic nvmet workqueue for work elements that
don't explicitly require a dedicated workqueue (by the mere fact
that they are using the system_wq).
Changes were done using the following replaces:
- s/schedule_work(/queue_work(nvmet_wq, /g
- s/schedule_delayed_work(/queue_delayed_work(nvmet_wq, /g
- s/flush_scheduled_work()/flush_workqueue(nvmet_wq)/g

The nvme-loop's admin queue may be freed and reallocated, and we have to
reset the flag of NVME_CTRL_ADMIN_Q_STOPPED so that the flag can match
with the quiesce state of the admin queue.
nvme-loop is the only driver to reallocate request queue, and not see
such usage in other nvme drivers.

We cannot detect a (perhaps buggy) controller that is sending us
a completion for a request that was already completed (for example
sending a completion twice), this phenomenon was seen in the wild
a few times.
So to protect against this, we use the upper 4 msbits of the nvme sqe
command_id to use as a 4-bit generation counter and verify it matches
the existing request generation that is incrementing on every execution.
The 16-bit command_id structure now is constructed by:

This means that we are giving up some possible queue depth as 12 bits
allow for a maximum queue depth of 4095 instead of 65536, however we
never create such long queues anyways so no real harm done.

During concurrent reset and delete calls the reset workqueue is
flushed, causing nvme_loop_reset_ctrl_work() to be executed when
the controller is in state DELETING or DELETING_NOIO.
But this is expected, so we shouldn't issue a WARN_ON here.

The queue count is increased in nvme_loop_init_io_queues(), so we
need to reset it to 1 at the end of nvme_loop_destroy_io_queues().
Otherwise the function is not re-entrant safe, and crash will happen
during concurrent reset and remove calls.

queue_rq() in pci only checks if the dispatched queue (nvmeq) is ready,
e.g. not being suspended. Since nvme_alloc_admin_tags() in reset flow
restarts the admin queue, users are able to submit admin commands to a
controller before reset_work() completes. Commands submitted under this
condition may interfere with commands that performs identify, IO queue
setup in reset_work(), and may result in a hang described in the
following patch.
As seen in the fabrics, user commands are prevented from being executed
under inproper controller states. We may reuse this logic to maintain a
clear admin queue during reset_work().

All nvme transport drivers preallocate an nvme command for each request.
Assume to use that command for nvme_setup_cmd() instead of requiring
drivers pass a pointer to it. All nvme drivers must initialize the
generic nvme_request 'cmd' to point to the transport's preallocated
nvme_command.
The generic nvme_request cmd pointer had previously been used only as a
temporary copy for passthrough commands. Since it now points to the
command that gets dispatched, passthrough commands must directly set it
up prior to executing the request.

This is a prep patch so that we can move the identify data structure
related code initialization from nvme_init_identify() into a helper.
Rename the function nvmet_init_identify() to nvmet_init_ctrl_finish().
Next patch will move the nvme_id_ctrl related initialization from newly
renamed function nvme_init_ctrl_finish() into the nvme_init_identify()
helper.

Fabrics drivers currently reserve two tags on the admin queue. But
given that the connect command is only run on a freshly created queue
or after all commands have been force aborted we only need to reserve
a single tag.

Set nvme-loop's lock class via blk_mq_hctx_set_fq_lock_class for avoiding
lockdep possible recursive locking, then we can remove the dynamically
allocated lock class for each flush queue, finally we can avoid horrible
SCSI probe delay.
This way may not address situation in which one nvme-loop is backed on
another nvme-loop. However, in reality, people seldom uses this way
for test. Even though someone played in this way, it is just one
recursive locking false positive, no real deadlock issue.

The function nvme_init_ctrl() gets the ctrl reference & when it fails it
does put the ctrl reference in the error unwind code.
When creating loop ctrl in nvme_loop_create_ctrl() if nvme_init_ctrl()
returns non zero (i.e. error) value it jumps to the "out_put_ctrl" label
which calls nvme_put_ctrl(), that will lead to douple ctrl put in error
unwind path.
Update nvme_loop_create_ctrl() such that this patch removes the
"out_put_ctrl" label, add a new "out" label after nvme_put_ctrl() in
error unwind path and jump to newly added label when nvme_init_ctrl()
call retuns an error.

nvme_end_request is a bit misnamed, as it wraps around the
blk_mq_complete_* API. It's semantics also are non-trivial, so give it
a more descriptive name and add a comment explaining the semantics.

When creating a loop controller (ctrl) in nvme_loop_create_ctrl() ->
nvme_init_ctrl() we set the ctrl state to NVME_CTRL_NEW.
Prior to [1] NVME_CTRL_NEW state was allowed in nvmf_check_ready() for
fabrics command type connect. Now, this fails in the following code path
for fabrics connect command when creating admin queue :-

so that the above mentioned check for nvmf_check_ready() will return
true.
This patch sets the ctrl state to connecting after we init the ctrl in
nvme_loop_create_ctrl()
nvme_init_ctrl() .

Put the ctrl reference count at nvme_uninit_ctrl as opposed to
nvme_init_ctrl which takes it. This decrease the reference count at the
core layer instead of decreasing it on each transport separately.
Also move the call of nvme_uninit_ctrl at PCI driver after calling to
nvme_release_prp_pools and nvme_dev_unmap, in order to put the reference
count after using the dev. This is safe because those functions use
nvme_dev which is freed only later at nvme_pci_free_ctrl.

In case nvme_sysfs_delete() is called by the user before taking the ctrl
reference count, the ctrl may be freed during the creation and cause the
bug. Take the reference as soon as the controller is externally visible,
which is done by cdev_device_add() in nvme_init_ctrl(). Also take the
reference count at the core layer instead of taking it on each transport
separately.

nvme_loop_create_io_queues() preallocates a big buffer for the IO SGL based
on SG_CHUNK_SIZE.
Modern DMA engines are often capable of dealing with very big segments so
the SG_CHUNK_SIZE is often too big. SG_CHUNK_SIZE results in a static 4KB
SGL allocation per command.
If a controller has lots of deep queues, preallocation for the sg list can
consume substantial amounts of memory. For nvmet-loop, nr_hw_queues can be
128 and each queue's depth 128. This means the resulting preallocation
for the data SGL is 128*128*4K = 64MB per controller.
Switch to runtime allocation for SGL for lists longer than 2 entries. This
is the approach used by NVMe PCI so it should be reasonable for NVMeOF as
well. Runtime SGL allocation has always been the case for the legacy I/O
path so this is nothing new.

nvme_cleanup_cmd should be called for each call to nvme_setup_cmd
(symmetrical functions). Move the call for nvme_cleanup_cmd to the common
core layer and call it during nvme_complete_rq for the good flow. For
error flow, each transport will call nvme_cleanup_cmd independently. Also
take care of a special case of path failure, where we call
nvme_complete_rq without doing nvme_setup_cmd.
