Adding vendor specific directories in phy to group
phy drivers under their respective vendor umbrella.
Also updated the MAINTAINERS file to reflect the correct
directory structure for phy drivers.

Current code uses num_phys settings to tell the number of entries in phys.
Thus remove the NULL terminating entry from phys array which is not necessary.

Add a new driver for the Exynos USB 2.0 PHY. The new driver uses the generic
PHY framework. The driver includes support for the Exynos 4x10 and 4x12
SoC families.
