There is no good reason to keep genhd.h separate from the main blkdev.h
header that includes it. So fold the contents of genhd.h into blkdev.h
and remove genhd.h entirely.

Suppose we have an environment with a number of non-NPIV FCP devices
(virtual HBAs / FCP devices / zfcp "adapter"s) sharing the same physical
FCP channel (HBA port) and its I_T nexus. Plus a number of storage target
ports zoned to such shared channel. Now one target port logs out of the
fabric causing an RSCN. Zfcp reacts with an ADISC ELS and subsequent port
recovery depending on the ADISC result. This happens on all such FCP
devices (in different Linux images) concurrently as they all receive a copy
of this RSCN. In the following we look at one of those FCP devices.
Requests other than FSF_QTCB_FCP_CMND can be slow until they get a
response.
Depending on which requests are affected by slow responses, there are
different recovery outcomes. Here we want to fix failed recoveries on port
or adapter level by avoiding recovery requests that can be slow.
We need the cached N_Port_ID for the remote port "link" test with ADISC.
Just before sending the ADISC, we now intentionally forget the old cached
N_Port_ID. The idea is that on receiving an RSCN for a port, we have to
assume that any cached information about this port is stale. This forces a
fresh new GID_PN [FC-GS] nameserver lookup on any subsequent recovery for
the same port. Since we typically can still communicate with the nameserver
efficiently, we now reach steady state quicker: Either the nameserver still
does not know about the port so we stop recovery, or the nameserver already
knows the port potentially with a new N_Port_ID and we can successfully and
quickly perform open port recovery. For the one case, where ADISC returns
successfully, we re-initialize port->d_id because that case does not
involve any port recovery.
This also solves a problem if the storage WWPN quickly logs into the fabric
again but with a different N_Port_ID. Such as on virtual WWPN takeover
during target NPIV failover.

RSCN from the storage FDISC was ignored by zfcp and we could not
successfully recover the failover. On some later failback on the storage,
we could have been lucky if the virtual WWPN got the same old N_Port_ID
from the SAN switch as we still had cached. Then the related RSCN
triggered a successful port reopen recovery. However, there is no
guarantee to get the same N_Port_ID on NPIV FDISC.
Even though NPIV-enabled FCP devices are not affected by this problem, this
code change optimizes recovery time for gone remote ports as a side effect.
The timely drop of cached N_Port_IDs prevents unnecessary slow open port
attempts.
While the problem might have been in code before v2.6.32 commit
799b76d09aee ("[SCSI] zfcp: Decouple gid_pn requests from erp") this fix
depends on the gid_pn_work introduced with that commit, so we mark it as
culprit to satisfy fix dependencies.
Note: Point-to-point remote port is already handled separately and gets its
N_Port_ID from the cached peer_d_id. So resetting port->d_id in general
does not affect PtP.

Pull dax and libnvdimm updates from Dan Williams:
"The bulk of this is a rework of the dax_operations API after
discovering the obstacles it posed to the work-in-progress DAX+reflink
support for XFS and other copy-on-write filesystem mechanics.
Primarily the need to plumb a block_device through the API to handle
partition offsets was a sticking point and Christoph untangled that
dependency in addition to other cleanups to make landing the
DAX+reflink support easier.
The DAX_PMEM_COMPAT option has been around for 4 years and not only
are distributions shipping userspace that understand the current
configuration API, but some are not even bothering to turn this option
on anymore, so it seems a good time to remove it per the deprecation
schedule. Recall that this was added after the device-dax subsystem
moved from /sys/class/dax to /sys/bus/dax for its sysfs organization.
All recent functionality depends on /sys/bus/dax.
Some other miscellaneous cleanups and reflink prep patches are
included as well.
Summary:
- Simplify the dax_operations API:
- Eliminate bdev_dax_pgoff() in favor of the filesystem
maintaining and applying a partition offset to all its DAX iomap
operations.
- Remove wrappers and device-mapper stacked callbacks for
->copy_from_iter() and ->copy_to_iter() in favor of moving
block_device relative offset responsibility to the
dax_direct_access() caller.
- Remove the need for an @bdev in filesystem-DAX infrastructure
- Remove unused uio helpers copy_from_iter_flushcache() and
copy_mc_to_iter() as only the non-check_copy_size() versions are
used for DAX.
- Prepare XFS for the pending (next merge window) DAX+reflink support
- Remove deprecated DEV_DAX_PMEM_COMPAT support
- Cleanup a straggling misuse of the GUID api"

These methods indirect the actual DAX read/write path. In the end pmem
uses magic flush and mc safe variants and fuse and dcssblk use plain ones
while device mapper picks redirects to the underlying device.
Add set_dax_nocache() and set_dax_nomc() APIs to control which copy
routines are used to remove indirect call from the read/write fast path
as well as a lot of boilerplate code.

Since commit a9c3f68f3cd8d (tty: Fix low_latency BUG) in 2014,
tty_flip_buffer_push() is only a wrapper to tty_schedule_flip(). We are
going to remove the latter (as it is used less), so call the former in
the rest of the users.

qeth_l2_detect_dev2br_support() will only set brport_hw_features for IQD
devices. So qeth_l2_bridge_getlink() and qeth_l2_bridge_setlink() will
always return -EOPNOTSUPP on OSA devices. Just don't offer these

Now that the OSN code is gone, we don't need the second switch statement
in the RX path. And getting rid of its (unreachable) default case is a
nice simplification.
Also don't pass in the full HW header, all we still need is a flag to
indicate whether the skb can use CSO. This we can already obtain during
the first peek at the HW header.

We always need an RX queue, and there's no reconfig situation either
where we would need to free & rebuild the queue.
So allocate the RX queue right from the start, and avoid freeing it
during unrelated qeth_free_qdio_queues() calls.

When sending a CCA CPRB to a control domain, the CPRB has to be sent via a
usage domain. Previous code used the default domain to route this message.
If the default domain is not online and ready to send the CPRB, the ioctl will
fail even if other usage domains could be used to send the CPRB.
To improve this, instead of using the default domain, switch to auto-select of
the domain.

This patch adds a sysfs 'status' attribute to a queue device when it is
bound to the vfio_ap device driver. The field displays a string indicating
the status of the queue device:
Status String: Indicates:
------------- ---------
"assigned" the queue is assigned to an mdev, but is not in use by a
KVM guest.
"in use" the queue is assigned to an mdev and is in use by a KVM
guest.
"unassigned" the queue is not assigned to an mdev.
The status string will be displayed by the 'lszcrypt' command if the queue
device is bound to the vfio_ap device driver.

All callers of set_buf_states() are already making sure that 'count'
is not 0. So don't check it an additional time.
Note that our own code also doesn't _require_ the count to be sane
(ie. we can't overrun an array or similar). So worst case HW would
simply reject the SQBS operation and report an error.

qdio_handle_activate_check() tries to re-use one of the queue-specific
handlers to report that the ACTIVATE ccw has been terminated. But the
logic to select that handler is overly complex - in practice both
qdio drivers have at least one Input Queue, so we never take the other
paths.
Make things more obvious by removing this unused code, and clearly
spelling out that we re-use the Input Handler for generic error
reporting. This also paves the way for a world without queue-specific
error handlers.

The qdio_irq contains only two fields that are directly exposed to the
HW (ccw and qib). And only the ccw needs to reside in 31-bit memory. So
allocate it separately, and remove the GFP_DMA constraint from the
qdio_irq allocation.

Fetch the individual CIWs when we actually need them, rather than
fetching both of them in qdio_setup_irq() and then needing to cache
them inside the qdio_irq.
Also deal with the error when a CIW is not available, instead of

commit fa1a8c23eb7d ("s390: cio: Delay uevents for subchannels")
introduced suppression of uevents for a subchannel until after it is
clear that the subchannel would not be unregistered again
immediately. This was done to avoid uevents being generated for I/O
subchannels with no valid device, which can happen on LPAR.
However, this also has some drawbacks: All subchannel drivers need to
manually remove the uevent suppression and generate an ADD uevent as
soon as they are sure that the subchannel will stay around. This misses
out on all uevents that are not the initial ADD uevent that would be
generated while uevents are suppressed; for example, all subchannels
were missing the BIND uevent.
As uevents being generated even for I/O subchannels without an
operational device turned out to be not as bad as missing uevents and
complicating the code flow, let's remove uevent suppression for
subchannels.

Pull s390 updates from Vasily Gorbik:
- Add support for ftrace with direct call and ftrace direct call

Slight rework of function __ap_revise_reserved()
because of unused variable warning when build with W=1.
This patch introduces an additional debug feature warning
message when device_reprobe() returns with failure.
However, the return value of __ap_revise_reserved()
is still hard coded to 0 as this is a callback function
to be used together with bus_for_each_dev() and thus
the return value indicates to go on with the
bus_for_each_dev() loop and not apport on a failure
of something within this function.

Since commit 48720ba56891 ("virtio/s390: use DMA memory for ccw I/O and
classic notifiers") we were supposed to make sure that
virtio_ccw_release_dev() completes before the ccw device and the
attached dma pool are torn down, but unfortunately we did not. Before
that commit it used to be OK to delay cleaning up the memory allocated
by virtio-ccw indefinitely (which isn't really intuitive for guys used
to destruction happens in reverse construction order), but now we
trigger a BUG_ON if the genpool is destroyed before all memory allocated
from it is deallocated. Which brings down the guest. We can observe this
problem, when unregister_virtio_device() does not give up the last
reference to the virtio_device (e.g. because a virtio-scsi attached scsi
disk got removed without previously unmounting its previously mounted
partition).
To make sure that the genpool is only destroyed after all the necessary
freeing is done let us take a reference on the ccw device on each
ccw_device_dma_zalloc() and give it up on each ccw_device_dma_free().
Actually there are multiple approaches to fixing the problem at hand
that can work. The upside of this one is that it is the safest one while
remaining simple. We don't crash the guest even if the driver does not
pair allocations and frees. The downside is the reference counting
overhead, that the reference counting for ccw devices becomes more
complex, in a sense that we need to pair the calls to the aforementioned
functions for it to be correct, and that if we happen to leak, we leak
more than necessary (the whole ccw device instead of just the genpool).
Some alternatives to this approach are taking a reference in
virtio_ccw_online() and giving it up in virtio_ccw_release_dev() or
making sure virtio_ccw_release_dev() completes its work before
virtio_ccw_remove() returns. The downside of these approaches is that
these are less safe against programming errors.

This patch introduces a new AP module option to be able to
control if the ap bus code is using interrupts or not.
By default if the interrupt support is available it is used.
This option makes it possible to disable interrupt use even
when interrupt support is available.
It should be obvious that this option can't magically enable
interrupt support when the hardware or hypervisor layer does
not support AP interrupts.
On the kernel command line use ap.useirq=0 or ap.useirq=1
to disable or enable (that's the default) interrupt use.

Provide physical addresses whenever the hardware interface
expects it or a 32-bit value used for tracking.
Variable sclp_early_sccb gets initialized in the decompressor
and points to an address in physcal memory. Yet, it is used
as virtual memory pointer and therefore should be converted.
Note, the other two __bootdata variables sclp_info_sccb and
sclp_info_sccb_valid contain plain data, but no pointers and
do need any special care.

This patch reworks all the debug feature invocations to be
more uniform. All invocations now use the macro with the
level already part of the macro name. All messages now start
with %s filled with __func__ (well there are still some
exceptions), and some message text has been shortened or
reworked.
There is no functional code touched with this patch.

When a queue is switched to soft offline during heavy load and later
switched to soft online again and now used, it may be that the caller
is blocked forever in the ioctl call.
The failure occurs because there is a pending reply after the queue(s)
have been switched to offline. This orphaned reply is received when
the queue is switched to online and is accidentally counted for the
outstanding replies. So when there was a valid outstanding reply and
this orphaned reply is received it counts as the outstanding one thus
dropping the outstanding counter to 0. Voila, with this counter the
receive function is not called any more and the real outstanding reply
is never received (until another request comes in...) and the ioctl
blocks.
The fix is simple. However, instead of readjusting the counter when an
orphaned reply is detected, I check the queue status for not empty and
compare this to the outstanding counter. So if the queue is not empty
then the counter must not drop to 0 but at least have a value of 1.
