"This is mostly update of the usual drivers: qla2xxx, hpsa, lpfc, ufs,
mpt3sas, ibmvscsi, megaraid_sas, bnx2fc and hisi_sas as well as the
removal of the osst driver (I heard from Willem privately that he
would like the driver removed because all his test hardware has
failed). Plus number of minor changes, spelling fixes and other
trivia.
The big merge conflict this time around is the SPDX licence tags.
Following discussion on linux-next, we believe our version to be more
accurate than the one in the tree, so the resolution is to take our
version for all the SPDX conflicts"
Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.
In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").
In these cases I picked the new-style one.
In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:
"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:
* This file is licensed under GPLv2.
In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"
So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.
Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.
Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

The reason that there was a mismatch is that the SCSI command is actual
returned from the cleanup handler. In previous testing, the type of
cleanup notification we'd get from the CQE did not trigger the code that
returned the SCSI command. To overcome the previous behavior we would put
a reference in bnx2fc_abts_cleanup() to account for the SCSI command.
However, in cases where the SCSI command is actually off, we end up with an
extra put.
The fix for this is to only take the extra put in bnx2fc_abts_cleanup if
the completion for the cleanup times out.

For bnx2fc, the source FCoE MAC is stored in the fcoe_port struct in the
data_src_mac field. Currently this is set in fcoe_ctlr_recv_flogi which
ends up setting it by simply using fc_fcoe_set_mac() which only uses the
default FCF-MAC. We still want to store the source FCoE MAC in
port->data_src_mac but we want to snoop the FLOGI response payload so as to
set it in the following method:
1. If a granted_mac is found, use that.
2. If not granted_mac is there but there is a FCF-MAP from the FCF then
create the MAC from the FCF-MAP and the destination ID from the frame.
3. If there is no FCF-MAP the use the spec. default FCF-MAP and the
destination ID from the frame.

Currently an int is being shifted and the result is being cast to a u64
which leads to undefined behaviour if the shift is more than 31 bits. Fix
this by casting the integer value 1 to u64 before the shift operation.

it, so there is no reason to call it from driver code. This patch was

NOTE: mmiowb() has only ever guaranteed ordering in conjunction with
spin_unlock(). However, pairing each mmiowb() removal in this patch with
the corresponding call to spin_unlock() is not at all trivial, so there
is a small chance that this change may regress any drivers incorrectly
relying on mmiowb() to order MMIO writes between CPUs using lock-free
synchronisation. If you've ended up bisecting to this commit, you can
reintroduce the mmiowb() calls using wmb() instead, which should restore
the old behaviour on all architectures other than some esoteric ia64
systems.

commit 1917d42d14b7 ("fcoe: use enum for fip_mode") introduces a separate
enum for the fip_mode that shall be used during initialisation handling
until it is passed to fcoe_ctrl_link_up to set the initial fip_state. That
change was incomplete and gcc quietly converted in various places between
the fip_mode and the fip_state enum values with implicit enum conversions,
which fortunately cannot cause any issues in the actual code's execution.
clang however warns about these implicit enum conversions in the scsi
drivers. This commit consolidates the use of the two enums, guided by
clang's enum-conversion warnings.
This commit now completes the use of the fip_mode: It expects and uses
fip_mode in {bnx2fc,fcoe}_interface_create and fcoe_ctlr_init, and it calls
fcoe_ctrl_set_set() with the correct values in fcoe_ctlr_link_up(). It
also breaks the association between FIP_MODE_AUTO and FIP_ST_AUTO to
indicate these two enums are distinct.

There are two issues here. First if cmgr->hba is not set early enough then
it leads to a NULL dereference. Second if we don't completely initialize
cmgr->io_bdt_pool[] then we end up dereferencing uninitialized pointers.

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:
kzalloc(a * b, gfp)
with:
kcalloc(a * b, gfp)
as well as handling cases of:
kzalloc(a * b * c, gfp)
with:
kzalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kzalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kzalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

When a request times out we set the io_req flag BNX2FC_FLAG_IO_COMPL so
that if a subsequent completion comes in on that task ID we will ignore
it. The issue is that in the check for this flag there is a missing
return so we will continue to process a request which may have already
been returned to the ownership of the SCSI layer. This can cause
unpredictable results.
Solution is to add in the missing return.

This converts all remaining cases of the old setup_timer() API into using
timer_setup(), where the callback argument is the structure already
holding the struct timer_list. These should have no behavioral changes,
since they just change which pointer is passed into the callback with
the same available pointers after conversion. It handles the following
examples, in addition to some other variations.
Casting from unsigned long:

If a cleanup task is not responded to while we are in bnx2fc_abts_cleanup, it
will hang the SCSI error handler since we use wait_for_completion instead of
wait_for_completion_timeout. So, use wait_for_completion_timeout so that we
don't hang the SCSI error handler thread forever.

The CPU hotplug related code of this driver can be simplified by:
1) Consolidating the callbacks into a single state. The CPU thread can be
torn down on the CPU which goes offline. There is no point in delaying
that to the CPU dead state
2) Let the core code invoke the online/offline callbacks and remove the
extra for_each_online_cpu() loops.

The BNX2FC module init/exit code installs/removes the hotplug callbacks with
the cpu hotplug lock held. This worked with the old CPU locking
implementation which allowed recursive locking, but with the new percpu
rwsem based mechanism this is not longer allowed.
Use the _cpuslocked() variants to fix this.

bnx2fc_process_new_cqes() has protection against CPU hotplug, which relies
on the per cpu thread pointer. This protection is racy because it happens
only partially with the per cpu fp_work_lock held.
If the CPU is unplugged after the lock is dropped, the wakeup code can
dereference a NULL pointer or access freed and potentially reused memory.
Restructure the code so the thread check and wakeup happens with the
fp_work_lock held.

In eh_abort, driver is calling scsi->done() for a IO for which cleanup is
pending. As the IO is outstanding with the firmware, it may do DMA
associated with the IO. This may lead to heap corruption.
Do not complete the IO for which cleanup is still pending. Return failure
from eh_abort and let the SCSI-ml retry the IO.

Add the following filters to bnx2fc_recv_frame():
1. Filter out invalid packets
- eth->dest_mac[3] matches FC frame's D_ID
2. Filter out packets that are not from our connected target
- In FIP_ST_ENABLED mode
- eth->src_mac matches fcoe_ctlr->dest_addr
3. Filter out packets where if d_id of the packet doesn't belong to
the device when one is already assigned a port_id, only then this
packet is dropped

It seems like a historic accident that these return unsigned char *,
and in many places that means casts are required, more often than not.
Make these functions (skb_put, __skb_put and pskb_put) return void *
and remove all the casts across the tree, adding a (u8 *) cast only
where the unsigned char pointer was used directly, all done with the

This patch fixes the bug by adding a mutex to serialize the calls to

<linux/sched/signal.h>
We are going to split <linux/sched/signal.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/signal.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.
