Pull workqueue changes from Tejun Heo:
"PREPARE_[DELAYED_]WORK() were used to change the work function of work
items without fully reinitializing it; however, this makes workqueue
consider the work item as a different one from before and allows the
work item to start executing before the previous instance is finished
which can lead to extremely subtle issues which are painful to debug.
The interface has never been popular. This pull request contains
patches to remove existing usages and kill the interface. As one of
the changes was routed during the last devel cycle and another
depended on a pending change in nvme, for-3.15 contains a couple merge
commits.
In addition, interfaces which were deprecated quite a while ago -
__cancel_delayed_work() and WQ_NON_REENTRANT - are removed too"

Tejun Heo has made WQ_NON_REENTRANT useless in the dbf2576e37
("workqueue: make all workqueues non-reentrant"). So remove its
usages and definition.
This patch doesn't introduce any behavior changes.
tj: minor description updates.

PREPARE_[DELAYED_]WORK() are being phased out. They have few users
and a nasty surprise in terms of reentrancy guarantee as workqueue
considers work items to be different if they don't have the same work
function.
fwtty_peer->work is multiplexed with multiple work functions.
Introduce fwserial_peer_workfn() which invokes fwtty_peer->workfn and
always use it as the work function and update the users to set the
->workfn field instead of overriding the work function using

usb_hub->init_work is multiplexed with multiple work functions;
however, the work item is never queued while in-flight, so we can
simply use INIT_DELAYED_WORK() before each queueing.

ps3_vuart wasn't overriding the work item with multiple work functions

set the work function. We can simply invoke INIT_WORK() with the work

the work item isn't in active use during rt2800usb_probe_hw(), this
doesn't cause any behavior difference.

sleep_on is known broken and going away. The atari_scsi driver is one of
two remaining users in the falcon_get_lock() function, which is a rather
crazy piece of code. This does not attempt to fix the driver's locking
scheme in general, but at least prevents falcon_get_lock from going to
sleep when no other thread holds the same lock or tries to get it,
and we no longer schedule with irqs disabled.

This patch adds a parameter 'auto_update' to the con3270
driver, causing the 'auto_update' feature to be disabled
if unset.
The 'auto_update' feature will cause the con3270 driver
to switch to the console view whenever new system messages
are displayed, which makes working on the 3270 terminal
awkward.

Limit the number of bits to the maximum number of cpus a machine
can have.
possible_cpu_mask typically will have more bits set than a machine
may physically have. This results in wasted memory during per-cpu
memory allocations, if the possible mask contains more cpus than
physically possible for a given configuration.

The maximum usable buffer size of the s390 debug feature (when using
the sprintf_view) is 11 * sizeof(long) (1 pointer for the format
string + 10 arguments). When a larger buffer size is specified the
additional memory is unused and wasted per debug entry. So reducing
the buffer size to its maximum (or to the actual buffer size used)
will make more precious debug feature space usable.
For pci_msg, chsc_msg, and cio_crw we use the additional usable dbf
space to reduce the number of allocated pages.

Drivers for ccw consoles use ccw_device_probe_console to receive
an initialized ccw device which is already enabled for interrupts.
After that the device driver does the initialization of its private
data. This can race with unsolicited interrupts which can happen
once the device is enabled for interrupts.
Split ccw_device_probe_console into ccw_device_create_console and
ccw_device_enable_console and reorder the initialization of the ccw
console drivers.
While at it mark these functions as __init.

ccw consoles are in use before they can be properly registered with
the driver core. For devices which are in use by a device driver we
rely on the ccw_device's pointer to the driver callbacks to be valid.
For ccw consoles this pointer is NULL until they are registered later
during boot and we dereferenced this pointer. This worked by
chance on 64 bit builds (cdev->drv was NULL but the optional callback
cdev->drv->path_event was also NULL by coincidence) and was unnoticed
until we received reports about boot failures on 31 bit systems.
Fix it by initializing the driver pointer for ccw consoles.

Pull x86 EFI changes from Ingo Molnar:
"The main changes:
- Add debug code to the dump EFI pagetable - Borislav Petkov
- Make 1:1 runtime mapping robust when booting on machines with lots
of memory - Borislav Petkov
- Move the EFI facilities bits out of 'x86_efi_facility' and into
efi.flags which is the standard architecture independent place to
keep EFI state, by Matt Fleming.
- Add 'EFI mixed mode' support: this allows 64-bit kernels to be
booted from 32-bit firmware. This needs a bootloader that supports
the 'EFI handover protocol'. By Matt Fleming"

The ARM EFI boot stub doesn't need to care about the efi_early
infrastructure that x86 requires in order to do mixed mode thunking. So
wrap everything up in an efi_call_early() macro.
This allows x86 to do the necessary indirection jumps to call whatever
firmware interface is necessary (native or mixed mode), but also allows
the ARM folks to mask the fact that they don't support relocation in the
boot stub and need to pass 'sys_table_arg' to every function.

It's not possible to dereference the EFI System table directly when
booting a 64-bit kernel on a 32-bit EFI firmware because the size of
pointers don't match.
In preparation for supporting the above use case, build a list of
function pointers on boot so that callers don't have to worry about
converting pointer sizes through multiple levels of indirection.

It makes more sense to set the feature flag in the success path of the
detection function than it does to rely on the caller doing it. Apart
from it being more logical to group the code and data together, it sets
a much better example for new EFI architectures.

Commit d8c6ad3184ca651 ("sched/idle, PPC: Remove redundant
cpuidle_idle_call()") reintroduced ppc64_runlatch_off/on() in the
pseries cpuidle backend driver. Hence the cleanup caused by the
commit "c0c4301c54adde05:pseries/cpuidle: Remove redundant call
to ppc64_runlatch_off() in cpu idle routines" in conjuction
with the commit d8c6ad3184ca651 causes a build failure.

The core idle loop now takes care of it. However a few things need
checking:
- Invocation of cpuidle_idle_call() in pseries_lpar_idle() happened
through arch_cpu_idle() and was therefore always preceded by a call
to ppc64_runlatch_off(). To preserve this property now that
cpuidle_idle_call() is invoked directly from core code, a call to
ppc64_runlatch_off() has been added to idle_loop_prolog() in
platforms/pseries/processor_idle.c.
- Similarly, cpuidle_idle_call() was followed by ppc64_runlatch_off()
so a call to the later has been added to idle_loop_epilog().
- And since arch_cpu_idle() always made sure to re-enable IRQs if they
were not enabled, this is now
done in idle_loop_epilog() as well.
The above was made in order to keep the execution flow close to the
original. I don't know if that was strictly necessary. Someone well
aquainted with the platform details might find some room for possible
optimizations.

The "int check" argument of lock_acquire() and held_lock->check are
misleading. This is actually a boolean: 2 means "true", everything
else is "false".
And there is no need to pass 1 or 0 to lock_acquire() depending on
CONFIG_PROVE_LOCKING, __lock_acquire() checks prove_locking at the
start and clears "check" if !CONFIG_PROVE_LOCKING.
Note: probably we can simply kill this member/arg. The only explicit
user of check => 0 is rcu_lock_acquire(), perhaps we can change it to
use lock_acquire(trylock =>, read => 2). __lockdep_no_validate means
check => 0 implicitly, but we can change validate_chain() to check
hlock->instance->key instead. Not to mention it would be nice to get
rid of lockdep_set_novalidate_class().

We should not be using static variable mousedev_mix in methods that can be
called before that singleton gets assigned. While at it let's add open and
close methods to mousedev structure so that we do not need to test if we
are dealing with multiplexor or normal device and simply call appropriate
method directly.

If a new (id == -1) ff effect was uploaded from userspace,
ff-core.c::input_ff_upload() will have assigned a positive number to the
new effect id. Currently, evdev.c::evdev_do_ioctl() will save this new id
to userspace, regardless of whether the upload succeeded or not.
On upload failure, this can be confusing because the dev->ff->effects[]
array will not contain an element at the index of that new effect id.
This patch fixes this by leaving the id unchanged after upload fails.
Note: Unfortunately applications should still expect changed effect id for
quite some time.

Olivier Bonvalet reported having repeated crashes due to a failed
assertion he was hitting in rbd_img_obj_callback():
Assertion failure in rbd_img_obj_callback() at line 2165:
rbd_assert(which >= img_request->next_completion);
With a lot of help from Olivier with reproducing the problem
we were able to determine the object and image requests had
already been completed (and often freed) at the point the
assertion failed.
There was a great deal of discussion on the ceph-devel mailing list
about this. The problem only arose when there were two (or more)
object requests in an image request, and the problem was always
seen when the second request was being completed.
The problem is due to a race in the window between setting the
"done" flag on an object request and checking the image request's
next completion value. When the first object request completes, it
checks to see if its successor request is marked "done", and if
so, that request is also completed. In the process, the image
request's next_completion value is updated to reflect that both
the first and second requests are completed. By the time the
second request is able to check the next_completion value, it
has been set to a value *greater* than its own "which" value,
which caused an assertion to fail.
Fix this problem by skipping over any completion processing
unless the completing object request is the next one expected.
Test only for inequality (not >=), and eliminate the bad
assertion.
