Pull iommu fixes from Joerg Roedel:
- Race condition fixes for the AMD IOMMU driver.
These are five patches fixing two race conditions around
increase_address_space(). The first race condition was around the
non-atomic update of the domain page-table root pointer and the
variable containing the page-table depth (called mode). This is fixed
now be merging page-table root and mode into one 64-bit field which
is read/written atomically.
The second race condition was around updating the page-table root
pointer and making it public before the hardware caches were flushed.
This could cause addresses to be mapped and returned to drivers which
are not reachable by IOMMU hardware yet, causing IO page-faults. This
is fixed too by adding the necessary flushes before a new page-table
root is published.
Related to the race condition fixes these patches also add a missing
domain_flush_complete() barrier to update_domain() and a fix to bail
out of the loop which tries to increase the address space when the
call to increase_address_space() fails.
Qian was able to trigger the race conditions under high load and
memory pressure within a few days of testing. He confirmed that he
has seen no issues anymore with the fixes included here.

Elsewhere in the file, there is a list_for_each_entry with
&vdev->resv_regions as the second argument, suggesting that
&vdev->resv_regions is the list head. So exchange the
arguments on the list_add call to put the list head in the
second argument.

The Device Table needs to be updated before the new page-table root
can be published in domain->pt_root. Otherwise a concurrent call to
fetch_pte might fetch a PTE which is not reachable through the Device
Table Entry.

The update_domain() function is expected to also inform the hardware
about domain changes. This needs a COMPLETION_WAIT command to be sent
to all IOMMUs which use the domain.

When increase_address_space() fails to allocate memory, alloc_pte()
will call it again until it succeeds. Do not loop forever while trying
to increase the address space and just return an error instead.

The 'pt_root' and 'mode' struct members of 'struct protection_domain'
need to be get/set atomically, otherwise the page-table of the domain
can get corrupted.
Merge the fields into one atomic64_t struct member which can be
get/set atomically.

When the controller is reconnecting, the host fails I/O and admin
commands as the host cannot reach the controller. ns scanning may
revalidate namespaces during that period and it is wrong to remove
namespaces due to these failures as we may hang (see 205da2434301).
One command that may fail is nvme_identify_ns_descs. Since we return
success due to having ns identify descriptor list optional, we continue
to compare ns identifiers in nvme_revalidate_disk, obviously fail and
return -ENODEV to nvme_validate_ns, which will remove the namespace.
Exactly what we don't want to happen.

It seems that for whatever reason, gcc-10 ends up not inlining a couple
of functions that used to be inlined before. Even if they only have one
single callsite - it looks like gcc may have decided that the code was
unlikely, and not worth inlining.
The code generation difference is harmless, but caused a few new section
mismatch errors, since the (now no longer inlined) function wasn't in
the __init section, but called other init functions:

In both cases there were trivially just a single callsite from another

This is tripped as a result of irqs being disabled during the call to
dma_free_coherent() by release_event_pool(). At this point in the code path
we have quiesced the adapter and it is overly paranoid to be holding the

Commit ed830385a2b1 ("scsi: ibmvfc: Avoid loss of all paths during SVC node
reboot") introduced a regression where when the client resets or re-enables
its CRQ with the hypervisor there is a chance that if the server side
doesn't issue its INIT handshake quick enough the client can issue an
Implicit Logout prior to doing an NPIV Login. The server treats this
scenario as a protocol violation and closes the CRQ on its end forcing the
client through a reset that gets the client host state and next host action
out of agreement leading to a BUG assert.

In NPIV environment, a NPIV host may use a queue pair created by base host
or other NPIVs, so the check for a queue pair created by this NPIV is not
correct, and can cause an abort to fail, which in turn means the NVME
command not returned. This leads to hang in nvme_fc layer in
nvme_fc_delete_association() which waits for all I/Os to be returned, which
is seen as hang in the application.

When multiple instances of the same MHI product are present in a system,
we can see a splat from mhi_create_devices() - "sysfs: cannot create
duplicate filename".
This is because the device names assigned to the MHI channel devices are
non-unique. They consist of the channel's name, and the channel's pipe
id. For identical products, each instance is going to have the same
set of channel (both in name and pipe id).
To fix this, we prepend the device name of the parent device that the
MHI channels belong to. Since different instances of the same product
should have unique device names, this makes the MHI channel devices for
each product also unique.
Additionally, remove the pipe id from the MHI channel device name. This
is an internal detail to the MHI product that provides little value, and
imposes too much device specific internal details to userspace. It is
expected that channel with a specific name (ie "SAHARA") has a specific
client, and it does not matter what pipe id that channel is enumerated on.
The pipe id is an internal detail between the MHI bus, and the hardware.
The client is not expected to make decisions based on the pipe id, and to
do so would require the client to have intimate knowledge of the hardware,
which is inappropiate as it may violate the layering provided by the MHI
bus. The limitation of doing this is that each product may only have one
instance of a channel by a unique name. This limitation is appropriate
given the usecases of MHI channels.

When reading or writing MHI registers, the core assumes that the physical
link is a memory mapped PCI link. This assumption may not hold for all
MHI devices. The controller knows what is the physical link (ie PCI, I2C,
SPI, etc), and therefore knows the proper methods to access that link.
The controller can also handle link specific error scenarios, such as
reading -1 when the PCI link went down.
Therefore, it is appropriate that the MHI core requests the controller to
make register accesses on behalf of the core, which abstracts the core
from link specifics, and end up removing an unnecessary assumption.

If the MHI core detects invalid data due to a PCI read, it calls into
the controller via link_status() to double check that the link is infact
down. All in all, this is pretty pointless, and racy. There are no good
reasons for this, and only drawbacks.
Its pointless because chances are, the controller is going to do the same
thing to determine if the link is down - attempt a PCI access and compare
the result. This does not make the link status decision any smarter.
Its racy because its possible that the link was down at the time of the
MHI core access, but then recovered before the controller access. In this
case, the controller will indicate the link is not down, and the MHI core
will precede to use a bad value as the MHI core does not attempt to retry
the access.
Retrying the access in the MHI core is a bad idea because again, it is
racy - what if the link is down again? Furthermore, there may be some
higher level state associated with the link status, that is now invalid
because the link went down.
The only reason why the MHI core could see "invalid" data when doing a PCI
access, that is actually valid, is if the register actually contained the
PCI spec defined sentinel for an invalid access. In this case, it is
arguable that the MHI implementation broken, and should be fixed, not
worked around.
Therefore, remove the link_status() callback before anyone attempts to
implement it.

Powerdown is necessary if mhi_sync_power_up fails due to a timeout, to
clean up the resources. Otherwise a BUG could be triggered when
attempting to clean up MSIs because the IRQ is still active from a
request_irq().

The AHB clock must be on for qcom_snps_hsphy_init() to be able to write
the initialization sequence to the hardware, so move the clock
enablement to phy init and exit.

This patch fixes a regression in 5.7-rc1+
In commit 8fe75cd4cddf ("phy: qcom-qusb2: Add generic QUSB2 V2
PHY support"), the change was made to add "qcom,qusb2-v2-phy"
as a generic compat string. However the change also removed
the "qcom,sdm845-qusb2-phy" compat string, which is documented
in the binding and already in use.
This patch re-adds the "qcom,sdm845-qusb2-phy" compat string
which allows the driver to continue to work with existing dts
entries such as found on the db845c.

The usb_get_maximum_speed() function is part of the usb-common module,
so enable it by selecting the corresponding Kconfig symbol.
While at it, also make sure to depend on USB_SUPPORT because USB_PHY
requires that. This can lead to Kconfig conflicts if USB_SUPPORT is not
enabled while attempting to enable PHY_TEGRA_XUSB.

This patch replaces function module_init() with subsys_initcall().
It is needed to ensure that the core module of the driver is
initialized before a component tries to register with the core. This

This reverts commit dca0b44957e5 ("regulator: Use
driver_deferred_probe_timeout for regulator_init_complete_work"),
as we ended up reverting the default deferred_probe_timeout
value back to zero, to preserve behavior with 5.6 we need to
decouple the regulator timeout which was previously 30 seconds.
This avoids breaking some systems that depend on the regulator
timeout but don't require the deferred probe timeout.

deferred_probe_timeout fires
In commit c8c43cee29f6 ("driver core: Fix
driver_deferred_probe_check_state() logic"), we set the default
driver_deferred_probe_timeout value to 30 seconds to allow for
drivers that are missing dependencies to have some time so that
the dependency may be loaded from userland after initcalls_done
is set.
However, Yoshihiro Shimoda reported that on his device that
expects to have unmet dependencies (due to "optional links" in
its devicetree), was failing to mount the NFS root.
In digging further, it seemed the problem was that while the
device properly probes after waiting 30 seconds for any missing
modules to load, the ip_auto_config() had already failed,
resulting in NFS to fail. This was due to ip_auto_config()
calling wait_for_device_probe() which doesn't wait for the
driver_deferred_probe_timeout to fire.
This patch tries to fix the issue by creating a waitqueue
for the driver_deferred_probe_timeout, and calling wait_event()
to make sure driver_deferred_probe_timeout is zero in
wait_for_device_probe() to make sure all the probing is
finished.
The downside to this solution is that kernel functionality that
uses wait_for_device_probe(), will block until the
driver_deferred_probe_timeout fires, regardless of if there is
any missing dependencies.
However, the previous patch reverts the default timeout value to
zero, so this side-effect will only affect users who specify a
driver_deferred_probe_timeout= value as a boot argument, where
the additional delay would be beneficial to allow modules to
load later during boot.
Thanks to Geert for chasing down that ip_auto_config was why NFS
was failing in this case!

In commit c8c43cee29f6 ("driver core: Fix
driver_deferred_probe_check_state() logic") and following
changes the logic was changes slightly so that if there is no
driver to match whats found in the dtb, we wait the sepcified
seconds for modules to be loaded by userland, and then timeout,
where as previously we'd print "ignoring dependency for device,
assuming no driver" and immediately return -ENODEV after
initcall_done.
However, in the timeout case (which previously existed but was
practicaly un-used without a boot argument), the timeout message
uses dev_WARN(). This means folks are now seeing a big backtrace
in their boot logs if there a entry in their dts that doesn't
have a driver.
To fix this, lets use dev_warn(), instead of dev_WARN() to match
the previous error path.

This patch addresses a regression in 5.7-rc1+
In commit c8c43cee29f6 ("driver core: Fix
driver_deferred_probe_check_state() logic"), we both cleaned up
the logic and also set the default driver_deferred_probe_timeout
value to 30 seconds to allow for drivers that are missing
dependencies to have some time so that the dependency may be
loaded from userland after initcalls_done is set.
However, Yoshihiro Shimoda reported that on his device that
expects to have unmet dependencies (due to "optional links" in
its devicetree), was failing to mount the NFS root.
In digging further, it seemed the problem was that while the
device properly probes after waiting 30 seconds for any missing
modules to load, the ip_auto_config() had already failed,
resulting in NFS to fail. This was due to ip_auto_config()
calling wait_for_device_probe() which doesn't wait for the
driver_deferred_probe_timeout to fire.
Fixing that issue is possible, but could also introduce 30
second delays in bootups for users who don't have any
missing dependencies, which is not ideal.
So I think the best solution to avoid any regressions is to
revert back to a default timeout value of zero, and allow
systems that need to utilize the timeout in order for userland
to load any modules that supply misisng dependencies in the dts
to specify the timeout length via the exiting documented boot
argument.
Thanks to Geert for chasing down that ip_auto_config was why NFS
was failing in this case!

When commit 8375e74f2bca ("driver core: Add fw_devlink kernel
commandline option") added fw_devlink, it didn't implement "permissive"
mode correctly.
That commit got the device links flags correct to make sure unprobed
suppliers don't block the probing of a consumer. However, if a consumer
is waiting for mandatory suppliers to register, that could still block a
consumer from probing.
This commit fixes that by making sure in permissive mode, all suppliers
to a consumer are treated as a optional suppliers. So, even if a
consumer is waiting for suppliers to register and link itself (using the
DL_FLAG_SYNC_STATE_ONLY flag) to the supplier, the consumer is never
blocked from probing.

It's currently the amba driver's responsibility to initialize the pointer,
dma_parms, for its corresponding struct device. The benefit with this
approach allows us to avoid the initialization and to not waste memory for
the struct device_dma_parameters, as this can be decided on a case by case
basis.
However, it has turned out that this approach is not very practical. Not
only does it lead to open coding, but also to real errors. In principle
callers of dma_set_max_seg_size() doesn't check the error code, but just
assumes it succeeds.
For these reasons, let's do the initialization from the common amba bus at
the device registration point. This also follows the way the PCI devices
are being managed, see pci_device_add().

When serial console has been assigned to ttyPS1 (which is serial1 alias)
console index is not updated property and pointing to index -1 (statically
initialized) which ends up in situation where nothing has been printed on
the port.
The commit 18cc7ac8a28e ("Revert "serial: uartps: Register own uart console
and driver structures"") didn't contain this line which was removed by
accident.

By directly using kfree() in different places we risk missing one if
it is switched to using vfree(), especially if the corresponding
vmalloc() is hidden away within a common abstraction.
Oh wait, that's exactly what happened here.
So let's fix this by creating a common abstraction for the free case
as well.

Currently we check to make sure there is no error state on the extcon
handle for VBUS when writing to the HS_PHY_GENCONFIG_2 register. When using
the USB role-switch API we still need to write to this register absent an
extcon handle.
This patch makes the appropriate update to ensure the write happens if
role-switching is true.
