Commit 01d0c698536f ("sr: implement ->free_disk to simplify refcounting")
refactored sr_block_open(), initialized one variable with a duplicate
assignment (probably an unintended copy & paste duplication) and turned one
error case into an early return, which makes the initialization of the
return variable needless.
So, simplify the local variable initialization in sr_block_open() to make
the code a bit more clear.
No functional change. No change in resulting object code.

Implement the ->free_disk method to to put struct scsi_disk when the last
gendisk reference count goes away. This removes the need to clear
->private_data and thus freeze the queue on unbind.

Call free_opal_dev from scsi_disk_release as the opal_dev field is accessed

Requiring every ULP to have the scsi_drive as first member of the
private data is rather fragile and not necessary anyway. Just use

It is inevitable to cause use-after-free on q->queue_hw_ctx between
queue_for_each_hw_ctx() and blk_mq_update_nr_hw_queues(). And converting
to xarray can fix the uaf, meantime code gets cleaner.
Prepare for converting q->queue_hctx_ctx into xarray, one thing is that
xa_for_each() can only accept 'unsigned long' as index, so changes type
of hctx index of queue_for_each_hw_ctx() into 'unsigned long'.

q->queue_hw_ctx is really one blk-mq internal structure for retrieving
hctx via its index, not supposed to be used by drivers. Meantime drivers
can get the tags structure easily from tagset.

Both ->start_time_ns and the RQF_IO_STAT are set when the request is
allocated using blk_mq_alloc_request by dm-mpath in blk_mq_rq_ctx_init.
The block layer also ensures ->start_time_ns is only set when actually
needed.

The code to stack blk-mq drivers is only used by dm-multipath, and
will preferably stay that way. Make it optional and only selected
by device mapper, so that the buildbots more easily catch abuses
like the one that slipped in in the ufs driver in the last merged
window. Another positive side effects is that kernel builds without
device mapper shrink a little bit as well.

Replace open coded bio_clone_fast implementations with the actual helper.
Note that the bio allocated as part of the dm_io structure in alloc_io
will only actually be used later in alloc_tio, making this earlier
cloning of the information safe.

__bio_clone_fast should also clone integrity and crypto data, as a clone
without those is incomplete. Right now the only caller that can actually
support crypto and integrity data (dm) does it manually for the one
callchain that supports these, but we better do it properly in the core.
Note that all callers except for the above mentioned one also don't need
to handle failure at all, given that the integrity and crypto clones are
based on mempool allocations that won't fail for sleeping allocations.

Pass the block_device and operation that we plan to use this bio for to
bio_alloc to optimize the assignment. NULL/0 can be passed, both for the
passthrough case on a raw request_queue and to temporarily avoid
refactoring some nasty code.
Also move the gfp_mask argument after the nr_vecs argument for a much
more logical calling convention matching what most of the kernel does.

The memory mapped in process_rdma is contiguous, so there is no need
to loop over bio_add_page. Remove rnbd_bio_map_kern and just open code
the bio allocation and mapping in the caller.
