Pass the block_device and operation that we plan to use this bio for to
bio_alloc to optimize the assignment. NULL/0 can be passed, both for the
passthrough case on a raw request_queue and to temporarily avoid
refactoring some nasty code.
Also move the gfp_mask argument after the nr_vecs argument for a much
more logical calling convention matching what most of the kernel does.

The section "19) Editor modelines and other cruft" in
Documentation/process/coding-style.rst clearly says, "Do not include any
of these in source files."
I recently receive a patch to explicitly add a new one.
Let's do treewide cleanups, otherwise some people follow the existing code
and attempt to upstream their favoriate editor setups.
It is even nicer if scripts/checkpatch.pl can check it.
If we like to impose coding style in an editor-independent manner, I think
editorconfig (patch [1]) is a saner solution.

The error handling in this function frees "reg" but it is still on the
"o2hb_all_regions" list so it will lead to a use after freew. Joseph Qi
points out that we need to clear the bit in the "o2hb_region_bitmap" as
well

Since snprintf() returns the would-be-output size instead of the actual
output size, the succeeding calls may go beyond the given buffer limit.
Fix it by replacing with scnprintf().

There is no need to check return value of debugfs_create functions, but
the last sweep through ocfs missed a number of places where this was
happening. There is also no need to save the individual dentries for the
debugfs files, as everything is can just be removed at once when the
directory is removed.
By getting rid of the file dentries for the debugfs entries, a bit of
local memory can be saved as well.

When calling debugfs functions, there is no need to ever check the
return value. The function can work or not, but the code logic should
never do something different based on this.
Also, because there is no need to save the file dentry, remove all of
the variables that were being saved, and just recursively delete the
whole directory when shutting down, saving a lot of logic and local
variables.

Reading heartbeat data from lowest node rather than from zero, in cases
where the node is not defined from zero, can reduce the number of sectors
read.

This reverts commit ba16ddfbeb9d ("ocfs2/o2hb: check len for
bio_add_page() to avoid getting incorrect bio").
In my testing, this patch introduces a problem that mkfs can't have
slots more than 16 with 4k block size.
And the original logic is safe actually with the situation it mentions
so revert this commit.

Pull block layer updates from Jens Axboe:
"This is the first pull request for 4.14, containing most of the code
changes. It's a quiet series this round, which I think we needed after
the churn of the last few series. This contains:
- Fix for a registration race in loop, from Anton Volkov.
- Overflow complaint fix from Arnd for DAC960.
- Series of drbd changes from the usual suspects.
- Conversion of the stec/skd driver to blk-mq. From Bart.
- A few BFQ improvements/fixes from Paolo.
- CFQ improvement from Ritesh, allowing idling for group idle.

This way we don't need a block_device structure to submit I/O. The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open. Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).
For the actual I/O path all that we need is the gendisk, which exists
once per block device. But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.
Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

Replace bi_error with a new bi_status to allow for a clear conversion.
Note that device mapper overloaded bi_error with a private value, which
we'll have to keep arround at least for now and thus propagate to a
proper blk_status_t value.

Configfs is the interface for ocfs2-tools to set configure to kernel and
$configfs_dir/cluster/$clustername/heartbeat/dead_threshold is the one
used to configure heartbeat dead threshold. Kernel has a default value
of it but user can set O2CB_HEARTBEAT_THRESHOLD in /etc/sysconfig/o2cb
to override it.
Commit 45b997737a80 ("ocfs2/cluster: use per-attribute show and store
methods") changed heartbeat dead threshold name while ocfs2-tools did
not, so ocfs2-tools won't set this configurable and the default value is
always used. So revert it.

ktime is a union because the initial implementation stored the time in
scalar nanoseconds on 64 bit machine and in a endianess optimized timespec
variant for 32bit machines. The Y2038 cleanup removed the timespec variant
and switched everything to scalar nanoseconds. The union remained, but
become completely pointless.
Get rid of the union and just keep ktime_t as simple typedef of type s64.
The conversion was done with coccinelle and some manual mopping up.

Pull block layer updates from Jens Axboe:
"This is the main block pull request this series. Contrary to previous
release, I've kept the core and driver changes in the same branch. We
always ended up having dependencies between the two for obvious
reasons, so makes more sense to keep them together. That said, I'll
probably try and keep more topical branches going forward, especially
for cycles that end up being as busy as this one.
The major parts of this pull request is:
- Improved support for O_DIRECT on block devices, with a small
private implementation instead of using the pig that is
fs/direct-io.c. From Christoph.
- Request completion tracking in a scalable fashion. This is utilized
by two components in this pull, the new hybrid polling and the
writeback queue throttling code.
- Improved support for polling with O_DIRECT, adding a hybrid mode
that combines pure polling with an initial sleep. From me.
- Support for automatic throttling of writeback queues on the block
side. This uses feedback from the device completion latencies to
scale the queue on the block side up or down. From me.
- Support from SMR drives in the block layer and for SD. From Hannes
and Shaun.
- Multi-connection support for nbd. From Josef.
- Cleanup of request and bio flags, so we have a clear split between
which are bio (or rq) private, and which ones are shared. From
Christoph.
- A set of patches from Bart, that improve how we handle queue
stopping and starting in blk-mq.
- Support for WRITE_ZEROES from Chaitanya.
- Lightnvm updates from Javier/Matias.
- Supoort for FC for the nvme-over-fabrics code. From James Smart.

Remove the WRITE_* and READ_SYNC wrappers, and just use the flags

CURRENT_TIME is not y2038 safe.
Use y2038 safe ktime_get_real_seconds() here for timestamps. struct
heartbeat_block's hb_seq and deletetion time are already 64 bits wide
and accommodate times beyond y2038.
Also use y2038 safe ktime_get_real_ts64() for on disk inode timestamps.
These are also wide enough to accommodate time64_t.

Sometimes io error is returned when storage is down for a while. Like
for iscsi device, stroage is made offline when session timeout, and this
will make all io return -EIO. For this case, nodes shouldn't do
negotiate timeout but should fence self. So let nodes fence self when
o2hb_do_disk_heartbeat return an error, this is the same behavior with
o2hb without negotiate timer.

This series of patches is to fix the issue that when storage down, all
nodes will fence self due to write timeout.
With this patch set, all nodes will keep going until storage back
online, except if the following issue happens, then all nodes will do as
before to fence self.
1. io error got
2. network between nodes down
3. nodes panic
This patch (of 6):
When storage down, all nodes will fence self due to write timeout. The
negotiate timer is designed to avoid this, with it node will wait until
storage up again.
Negotiate timer working in the following way:
1. The timer expires before write timeout timer, its timeout is half
of write timeout now. It is re-queued along with write timeout timer.
If expires, it will send NEGO_TIMEOUT message to master node(node with
lowest node number). This message does nothing but marks a bit in a
bitmap recording which nodes are negotiating timeout on master node.
2. If storage down, nodes will send this message to master node, then
when master node finds its bitmap including all online nodes, it sends
NEGO_APPROVL message to all nodes one by one, this message will
re-queue write timeout timer and negotiate timer. For any node doesn't
receive this message or meets some issue when handling this message, it
will be fenced. If storage up at any time, o2hb_thread will run and
re-queue all the timer, nothing will be affected by these two steps.

PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.
This promise never materialized. And unlikely will.
We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE. And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.
Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.
Let's stop pretending that pages in page cache are special. They are
not.
The changes are pretty straight-forward:

- <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

This patch contains automated changes generated with coccinelle using
script below. For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.
The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.
There are few places in the code where coccinelle didn't reach. I'll
fix them manually in a separate patch. Comments and documentation also
will be addressed with the separate patch.

There actually no hardware or software interrupts in the context which
using o2hb_live_lock, so we don't need to worry about race conditions
caused by irq/softirq with spinlock held. Turning off irq is not good
for system performance after all. Just replace them with a non
interrupt safe function.

o2hb_region_release currently doesn't free o2hb_debug_buf
hr_db_elapsed_time and hr_db_pinned malloced in o2hb_debug_create. Also
we should call debugfs_remove before freeing its data, to prevent the risk
accessing debugfs rightly after its data has been freed.

When run multiple xattr test of ocfs2-test on a three-nodes cluster,
mount failed sometimes with the following message.
o2hb: Unable to stabilize heartbeart on region D18B775E758D4D80837E8CF3D086AD4A (xvdb)
Stabilize heartbeat depends on the timing order to mount ocfs2 from
cluster nodes and how fast the tcp connections are established. So
increase unsteady interations to leave more time for it.

Pull SCSI target updates from Nicholas Bellinger:
"This series contains HCH's changes to absorb configfs attribute
->show() + ->store() function pointer usage from it's original
tree-wide consumers, into common configfs code.
It includes usb-gadget, target w/ drivers, netconsole and ocfs2
changes to realize the improved simplicity, that now renders the
original include/target/configfs_macros.h CPP magic for fabric drivers
and others, unnecessary and obsolete.
And with common code in place, new configfs attributes can be added
easier than ever before.
Note, there are further improvements in-flight from other folks for
v4.5 code in configfs land, plus number of target fixes for post -rc1
code"
In the meantime, a new user of the now-removed old configfs API came in
through the char/misc tree in commit 7bd1d4093c2f ("stm class: Introduce
an abstraction for System Trace Module devices").
This merge resolution comes from Alexander Shishkin, who updated his stm
class tracing abstraction to account for the removal of the old
show_attribute and store_attribute methods in commit 517982229f78
("configfs: remove old API") from this pull. As Alexander says about
that patch:
"There's no need to keep an extra wrapper structure per item and the
awkward show_attribute/store_attribute item ops are no longer needed.
This patch converts policy code to the new api, all the while making
the code quite a bit smaller and easier on the eyes.

There is a race case between mount and delete node/cluster, which will
lead o2hb_thread to malfunctioning dead loop.

<<<<<< race window, node may have already been deleted, and then
enter the loop, o2hb thread will be malfunctioning
because of no configured nodes found.

So check the return value of o2nm_depend_this_node() is needed. If node
has been deleted, do not enter the loop and let mount fail.

o2hb_elapsed_msecs computes the time taken for a disk heartbeat.
'struct timeval' variables are used to store start and end times. On
32-bit systems, the 'tv_sec' component of 'struct timeval' will overflow
in year 2038 and beyond.
This patch solves the overflow with the following:
1. Replace o2hb_elapsed_msecs using 'ktime_t' values to measure start
and end time, and built-in function 'ktime_ms_delta' to compute the
elapsed time. ktime_get_real() is used since the code prints out the
wallclock time.
2. Changes format string to print time as a single 64-bit nanoseconds
value ("%lld") instead of seconds and microseconds. This simplifies
the code since converting ktime_t to that format would need expensive
computation. However, the debug log string is less readable than the
previous format.

'o2hb_map_slot_data' and 'o2hb_populate_slot_data' are called from only
one place, in 'o2hb_region_dev_write'. Return value is checked and
'mlog_errno' is called to log a message if it is not 0.
So there is no need to call 'mlog_errno' directly within these functions.
This would result on logging the message twice.
