One of the disadvantages of on-stack plugging is that we potentially
lose out on merging since all pending IO isn't always visible to
everybody. When we flush the on-stack plugs, right now we don't do
any checks to see if potential merge candidates could be utilized.
Correct this by adding a new insert variant, ELEVATOR_INSERT_SORT_MERGE.
It works just ELEVATOR_INSERT_SORT, but first checks whether we can
merge with an existing request before doing the insertion (if we fail
merging).
This fixes a regression with multiple processes issuing IO that
can be merged.

With the plugging now being explicitly controlled by the
submitter, callers need not pass down unplugging hints
to the block layer. If they want to unplug, it's because they
manually plugged on their own - in which case, they should just
unplug at will.

Code has been converted over to the new explicit on-stack plugging,
and delay users have been converted to use the new API for that.
So lets kill off the old plugging along with aops->sync_page().

This patch adds support for creating a queuing context outside
of the queue itself. This enables us to batch up pieces of IO
before grabbing the block device queue lock and submitting them to
the IO scheduler.
The context is created on the stack of the process and assigned in
the task structure, so that we can auto-unplug it if we hit a schedule
event.
The current queue plugging happens implicitly if IO is submitted to
an empty device, yet callers have to remember to unplug that IO when
they are going to wait for it. This is an ugly API and has caused bugs
in the past. Additionally, it requires hacks in the vm (->sync_page()
callback) to handle that logic. By switching to an explicit plugging
scheme we make the API a lot nicer and can get rid of the ->sync_page()
hack in the vm.

This merge creates two set of conflicts. One is simple context
conflicts caused by removal of throtl_scheduled_delayed_work() in
for-linus and removal of throtl_shutdown_timer_wq() in
for-2.6.39/core.
The other is caused by commit 255bb490c8 (block: blk-flush shouldn't
call directly into q->request_fn() __blk_run_queue()) in for-linus
crashing with FLUSH reimplementation in for-2.6.39/core. The conflict
isn't trivial but the resolution is straight-forward.
* __blk_run_queue() calls in flush_end_io() and flush_data_end_io()
should be called with @force_kblockd set to %true.
* elv_insert() in blk_kick_flush() should use
%ELEVATOR_INSERT_REQUEUE.
Both changes are to avoid invoking ->request_fn() directly from
request completion path and closely match the changes in the commit
255bb490c8.

If we enable trace events to trace block actions, We use
blk_fill_rwbs_rq to analyze the corresponding actions
in request's cmd_flags, but we only choose the minor 2 bits
from it, so most of other flags(e.g, REQ_SYNC) are missing.
For example, with a sync write we get:
write_test-2409 [001] 160.013869: block_rq_insert: 3,64 W 0 () 258135 + =
8 [write_test]
Since now we have integrated the flags of both bio and request,
it is safe to pass rq->cmd_flags directly to blk_fill_rwbs and
blk_fill_rwbs_rq isn't needed any more.
With this patch, after a sync write we get:
write_test-2417 [000] 226.603878: block_rq_insert: 3,64 WS 0 () 258135 +=
8 [write_test]

__blk_run_queue() automatically either calls q->request_fn() directly
or schedules kblockd depending on whether the function is recursed.
blk-flush implementation needs to be able to explicitly choose
kblockd. Add @force_kblockd.
All the current users are converted to specify %false for the
parameter and this patch doesn't introduce any behavior change.

o Dominik Klein reported a system hang issue while doing some blkio
throttling testing.
https://lkml.org/lkml/2011/2/24/173
o Some tracing revealed that CFQ was not dispatching any more jobs as
queue unplug was not happening. And queue unplug was not happening
because unplug work was not being called as there was one throttling
work on same cpu which as not finished yet. And throttling work had not
finished as it was tyring to dispatch a bio to CFQ but all the request
descriptors were consume to it was put to sleep.
o So basically it is a cyclic dependecny between CFQ unplug work and
throtl dispatch work. Tejun suggested that use separate workqueue for
such cases.
o This patch uses a separate workqueue for throttle related work and
does not rely on kblockd workqueue anymore.

Several ACPI drivers fail to build if CONFIG_NET is unset, because
they refer to things depending on CONFIG_THERMAL that in turn depends
on CONFIG_NET. However, CONFIG_THERMAL doesn't really need to depend
on CONFIG_NET, because the only part of it requiring CONFIG_NET is
the netlink interface in thermal_sys.c.
Put the netlink interface in thermal_sys.c under #ifdef CONFIG_NET
and remove the dependency of CONFIG_THERMAL on CONFIG_NET from
drivers/thermal/Kconfig.

Commit e2cda3226481 ("thp: add pmd mangling generic functions") replaced
some macros in <asm-generic/pgtable.h> with inline functions.
If the functions are to be defined (not all architectures need them)
then struct vm_area_struct must be defined first. So include
<linux/mm_types.h>.

Commit 074037e (PM / Wakeup: Introduce wakeup source objects and
event statistics (v3)) caused ACPI wakeup to only work if
CONFIG_PM_SLEEP is set, but it also worked for CONFIG_PM_SLEEP unset

device_init_wakeup() and device_may_wakeup() work in the same way
as before commit 074037e when CONFIG_PM_SLEEP is unset.

There are two cases when we call flush_disk.
In one, the device has disappeared (check_disk_change) so any
data will hold becomes irrelevant.
In the oter, the device has changed size (check_disk_size_change)
so data we hold may be irrelevant.
In both cases it makes sense to discard any 'clean' buffers,
so they will be read back from the device if needed.
In the former case it makes sense to discard 'dirty' buffers
as there will never be anywhere safe to write the data. In the
second case it *does*not* make sense to discard dirty buffers
as that will lead to file system corruption when you simply enlarge
the containing devices.
flush_disk calls __invalidate_devices.
__invalidate_device calls both invalidate_inodes and invalidate_bdev.
invalidate_inodes *does* discard I_DIRTY inodes and this does lead
to fs corruption.
invalidate_bev *does*not* discard dirty pages, but I don't really care
about that at present.
So this patch adds a flag to __invalidate_device (calling it
__invalidate_device2) to indicate whether dirty buffers should be
killed, and this is passed to invalidate_inodes which can choose to
skip dirty inodes.
flusk_disk then passes true from check_disk_change and false from
check_disk_size_change.
dm avoids tripping over this problem by calling i_size_write directly
rathher than using check_disk_size_change.
md does use check_disk_size_change and so is affected.
This regression was introduced by commit 608aeef17a which causes
check_disk_size_change to call flush_disk, so it is suitable for any
kernel since 2.6.27.

Michael Leun reported that running parallel opens on a fuse filesystem
can trigger a "kernel BUG at mm/truncate.c:475"
Gurudas Pai reported the same bug on NFS.
The reason is, unmap_mapping_range() is not prepared for more than
one concurrent invocation per inode. For example:
thread1: going through a big range, stops in the middle of a vma and
stores the restart address in vm_truncate_count.
thread2: comes in with a small (e.g. single page) unmap request on
the same vma, somewhere before restart_address, finds that the
vma was already unmapped up to the restart address and happily
returns without doing anything.
Another scenario would be two big unmap requests, both having to
restart the unmapping and each one setting vm_truncate_count to its
own value. This could go on forever without any of them being able to
finish.
Truncate and hole punching already serialize with i_mutex. Other
callers of unmap_mapping_range() do not, and it's difficult to get
i_mutex protection for all callers. In particular ->d_revalidate(),
which calls invalidate_inode_pages2_range() in fuse, may be called
with or without i_mutex.
This patch adds a new mutex to 'struct address_space' to prevent
running multiple concurrent unmap_mapping_range() on the same mapping.
[ We'll hopefully get rid of all this with the upcoming mm
preemptibility series by Peter Zijlstra, the "mm: Remove i_mmap_mutex
lockbreak" patch in particular. But that is for 2.6.39 ]

Assigning a socket in timewait state to skb->sk can trigger
kernel oops, e.g. in nfnetlink_log, which does:
if (skb->sk) {
read_lock_bh(&skb->sk->sk_callback_lock);
if (skb->sk->sk_socket && skb->sk->sk_socket->file) ...
in the timewait case, accessing sk->sk_callback_lock and sk->sk_socket
is invalid.
Either all of these spots will need to add a test for sk->sk_state != TCP_TIME_WAIT,
or xt_TPROXY must not assign a timewait socket to skb->sk.
This does the latter.
If a TW socket is found, assign the tproxy nfmark, but skip the skb->sk assignment,
thus mimicking behaviour of a '-m socket .. -j MARK/ACCEPT' re-routing rule.
The 'SYN to TW socket' case is left unchanged -- we try to redirect to the
listener socket.

Move blk_throtl_exit() in blk_cleanup_queue() as blk_throtl_exit() is
written in such a way that it needs queue lock. In blk_release_queue()
there is no gurantee that ->queue_lock is still around.
Initially blk_throtl_exit() was in blk_cleanup_queue() but Ingo reported
one problem.
https://lkml.org/lkml/2010/10/23/86
And a quick fix moved blk_throtl_exit() to blk_release_queue().
commit 7ad58c028652753814054f4e3ac58f925e7343f4
Author: Jens Axboe <jaxboe@fusionio.com>

block: fix use-after-free bug in blk throttle code
This patch reverts above change and does not try to shutdown the
throtl work in blk_sync_queue(). By avoiding call to
throtl_shutdown_timer_wq() from blk_sync_queue(), we should also avoid
the problem reported by Ingo.
blk_sync_queue() seems to be used only by md driver and it seems to be
using it to make sure q->unplug_fn is not called as md registers its
own unplug functions and it is about to free up the data structures
used by unplug_fn(). Block throttle does not call back into unplug_fn()
or into md. So there is no need to cancel blk throttle work.
In fact I think cancelling block throttle work is bad because it might
happen that some bios are throttled and scheduled to be dispatched later
with the help of pending work and if work is cancelled, these bios might
never be dispatched.
Block layer also uses blk_sync_queue() during blk_cleanup_queue() and
blk_release_queue() time. That should be safe as we are also calling
blk_throtl_exit() which should make sure all the throttling related
data structures are cleaned up.

Uwe pointed out that my alarm based UIE emulation is not sufficient
to replace the older timer/polling based UIE emulation on devices
where there is no alarm irq. This causes rtc devices without alarms
to return -EINVAL to UIE ioctls. The fix is to re-instate the old
timer/polling method for devices without alarm irqs.
This patch reverts the following commits:
042620a018afcfba1d678062b62e46 - Remove UIE emulation
1daeddd5962acad1bea55e524fc0fa - Cleanup removed UIE emulation declaration
b5cc8ca1c9c3a37eaddf709b2fd3e1 - Remove Kconfig symbol for UIE emulation
The emulation mode will still need to be wired-in with a following
patch before it will work.

mostly used by workqueue and in a few other odd places. Unify the

When list debugging is enabled, we aim to readably show list corruption
errors, and the basic list_add/list_del operations end up having extra
debugging code in them to do some basic validation of the list entries.
However, "list_del_init()" and "list_move[_tail]()" ended up avoiding
the debug code due to how they were written. This fixes that.
So the _next_ time we have list_move() problems with stale list entries,
we'll hopefully have an easier time finding them..

This patch removes the legacy procfs based target_core_mib.c code,
and moves the necessary scsi_index_tables functions and defines into
target_core_transport.c and target_core_base.h code to allow existing
fabric independent statistics to function.
This includes the removal of a handful of 'atomic_t mib_ref_count'
counters used in struct se_node_acl, se_session and se_hba to prevent
removal while using seq_list procfs walking logic.

Always allow backtraces when using oprofile on ARM, even if a PMU
isn't present. Restores functionality originally introduced in commit
1b7b56982fdcd9d85effd76f3928cf5d6eb26155 ("oprofile: Always allow
backtraces on ARM") by Richard Purdie.
It is not that obvious, but there is now only one oprofile_arch_init()
function. So the .backtrace callback is available also in timer mode.
Implemented by removing code and using stubs for oprofile_perf_{init,
exit} provided by <linux/oprofile.h>. This allows cleaning of other
architecture specific implementations too.
