led_blink_set[_oneshot]()'s delay_on and delay_off function parameters
are pass by reference, so that hw-blink implementations can report
back the actual achieved delays when the values have been rounded
to something the hw supports.
This is really only interesting for the sysfs API / the timer trigger.
Other triggers don't really care about this and none of the callers of
led_trigger_blink[_oneshot]() do anything with the returned delay values.
Change the led_trigger_blink[_oneshot]() delay parameters to pass-by-value,
there are 2 reasons for this:
1. led_cdev->blink_set() may sleep, while led_trigger_blink() may not.
So on hw where led_cdev->blink_set() sleeps the call needs to be deferred
to a workqueue, in which case the actual achieved delays are unknown
(this is a preparation patch for the deferring).
2. Since the callers don't care about the actual achieved delays, allowing
callers to directly pass a value leads to simpler code for most callers.

We shouldn't open-code led_trigger_blink() or led_trigger_event(),
use them instead of badly open-coding them. This also fixes the
locking, led_trigger_blink() and led_trigger_event() now use
read_lock_irqsave().

Given that activating a trigger can fail, let the callback return an
indication. This prevents to have a trigger active according to the
"trigger" sysfs attribute but not functional.
All users are changed accordingly to return 0 for now. There is no intended
change in behaviour.

As I was testing with hwsim, I missed that my previous commit to
make LED work depend on activation broke the code because I missed
removing the old trigger struct and some code was still using it,
now erroneously, causing crashes.
Fix this by always using the correct struct.

When LED triggers are compiled in, but not used, mac80211 will still
call them to update the status. This isn't really a problem for the
assoc and radio ones, but the TX/RX (and to a certain extend TPT)
ones can be called very frequently (for every packet.)
In order to avoid that when they're not used, track their activation
and call the corresponding trigger (and in the TPT case, account for
throughput) only when the trigger is actually used by an LED.
Additionally, make those trigger functions inlines since theyre only
used once in the remaining code.

Change mac80211 LED trigger code to use the generic
led_trigger_blink_oneshot() API for transmit and receive activity
indication.
This gives a better feedback to the user, as with the new API each
activity event results in a visible blink, while a constant traffic
results in a continuous blink at constant rate.

Rename leds external interface led_brightness_set() to led_set_brightness().
This is the second phase of the change to reduce confusion between the
leds internal and external interfaces that set brightness. With this change,
now the external interface is led_set_brightness(). The first phase renamed
the internal interface led_set_brightness() to __led_set_brightness().
There are no changes to the interface implementations.

These files are non modular, but need to export symbols using
the macros now living in export.h -- call out the include so
that things won't break when we remove the implicit presence
of module.h from everywhere.

The throughput LED trigger was always active when
the radio was enabled. In most cases that's likely
the desired behaviour, but iwlwifi requires it to
be only active when one of the virtual interfaces
is actually "connected" in some way.

iwlwifi and other drivers like to blink their LED
based on throughput. Implement this generically in
mac80211, based on a throughput table the driver
specifies. That way, drivers can set the blink
frequencies depending on their desired behaviour
and max throughput.
All the drivers need to do is provide an LED class
device, best with blink hardware offload.

The throughput trigger will require doing LED
classdev/trigger handling before register_hw(),
so drivers should have access to the trigger
names before it. If trigger registration fails,
this will still make the trigger name available,
but that's not a big problem since the default
trigger will the simply not be found.

implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.
