Using mapped writes, it's technically possible to expose stale
post-eof data on a truncate up operation. Consider the following
example:
$ xfs_io -fc "pwrite 0 2k" -c "mmap 0 4k" -c "mwrite 2k 2k" \
-c "truncate 8k" -c "pread -v 2k 16" <file>
...

This shows that the post-eof data written via mwrite lands within
EOF after a truncate up. While this is deliberate of the test case,
behavior is somewhat unpredictable because writeback does post-eof
zeroing, and writeback can occur at any time in the background. For
example, an fsync inserted between the mwrite and truncate causes
the subsequent read to instead return zeroes. This basically means
that there is a race window in this situation between any subsequent
extending operation and writeback that dictates whether post-eof
data is exposed to the file or zeroed.
To prevent this problem, perform partial block zeroing as part of
the various inode size extending operations that are susceptible to
it. For truncate extension, zero around the original eof similar to
how truncate down does partial zeroing of the new eof. For extension
via writes and fallocate related operations, zero the newly exposed
range of the file to cover any partial zeroing that must occur at
the original and new eof blocks.

When I enabled ext4 debug for fault injection testing, I encountered the
following warning:

The root cause of the issue lies in the improper implementation of ext4's
buffer_head read fault injection. The actual completion of buffer_head
read and the buffer_head fault injection are not atomic, which can lead
to the uptodate flag being cleared on normally used buffer_heads in race
conditions.

The best approach would be to perform fault injection in the IO completion
callback function, rather than after IO completion. However, the IO
completion callback function cannot get the fault injection code in sb.
Fix it by passing the result of fault injection into the bh read function,
we simulate faults within the bh read function itself. This requires adding
an extra parameter to the bh read functions that need fault injection.

When converting a delalloc extent in ext4_es_insert_extent(), since we
only want to pass the info of whether the quota has already been claimed
if the allocation is a direct allocation from ext4_map_create_blocks(),
there is no need to pass full mapping flags, so changes to just pass
whether the EXT4_GET_BLOCKS_DELALLOC_RESERVE bit is set.

The ext4_find_extent() can update the extent path so that it does not have
to allocate and free the path repeatedly, thus reducing the consumption of
memory allocation and freeing in the following functions:
ext4_ext_clear_bb
ext4_ext_replay_set_iblocks
ext4_fc_replay_add_range
ext4_fc_set_bitmaps_and_counters
No functional changes. Note that ext4_find_extent() does not support error
pointers, so in this case set path to NULL first.

Even though ext4_find_extent() returns an error, ext4_insert_range() still
returns 0. This may confuse the user as to why fallocate returns success,
but the contents of the file are not as expected. So propagate the error
returned by ext4_find_extent() to avoid inconsistencies.

When calling ext4_force_split_extent_at() in ext4_ext_replay_update_ex(),
the 'ppath' is updated but it is the 'path' that is freed, thus potentially
triggering a double-free in the following process:

In ext4_find_extent(), if the path is not big enough, we free it and set
*orig_path to NULL. But after reallocating and successfully initializing
the path, we don't update *orig_path, in which case the caller gets a
valid path but a NULL ppath, and this may cause a NULL pointer dereference
or a path memory leak. For example:

In ext4_find_extent(), path may be freed by error or be reallocated, so
using a previously saved *ppath may have been freed and thus may trigger
use-after-free, as follows:

Similar to ext4_split_extent_at(), we use *ppath directly as an input to
ext4_ext_show_leaf(). Fix a spelling error by the way.
Same problem in ext4_ext_handle_unwritten_extents(). Since 'path' is only
used in ext4_ext_show_leaf(), remove 'path' and use *ppath directly.
This issue is triggered only when EXT_DEBUG is defined and therefore does
not affect functionality.

In ext4_ext_rm_idx() and ext4_ext_correct_indexes(), there is no proper
rollback of already executed updates when updating a level of the extents
path fails, so we may get an inconsistent extents tree, which may trigger
some bad things in errors=continue mode.
Hence clear the verified bit of modified extents buffers if the tree fails
to be updated in ext4_ext_rm_idx() or ext4_ext_correct_indexes(), which
forces the extents buffers to be checked in ext4_valid_extent_entries(),
ensuring that the extents tree is consistent.

Now that we update data reserved space for delalloc after allocating
new blocks in ext4_{ind|ext}_map_blocks(), and if bigalloc feature is
enabled, we also need to query the extents_status tree to calculate the
exact reserved clusters. This is complicated now and it appears that
it's better to do this job in ext4_es_insert_extent(), because
__es_remove_extent() have already count delalloc blocks when removing
delalloc extents and __revise_pending() return new adding pending count,
we could update the reserved blocks easily in ext4_es_insert_extent().
We direct reduce the reserved cluster count when replacing a delalloc
extent. However, thers are two special cases need to concern about the
quota claiming when doing direct block allocation (e.g. from fallocate).
A),
fallocate a range that covers a delalloc extent but start with
non-delayed allocated blocks, e.g. a hole.

^^^^^^^^^^^^^^^^^^^^^^^ fallocate this range
Current ext4_map_blocks() can't always trim the extent since it may
release i_data_sem before calling ext4_map_create_blocks() and raced by
another delayed allocation. Hence the EXT4_GET_BLOCKS_DELALLOC_RESERVE
may not set even when we are replacing a delalloc extent, without this
flag set, the quota has already been claimed by ext4_mb_new_blocks(), so
we should release the quota reservations instead of claim them again.
B),
bigalloc feature is enabled, fallocate a range that contains non-delayed
allocated blocks.

The max_zeroout is of type int and the s_extent_max_zeroout_kb is of
type uint, and the s_extent_max_zeroout_kb can be freely modified via
the sysfs interface. When the block size is 1024, max_zeroout may
overflow, so declare it as unsigned int to avoid overflow.

In ext4_map_blocks(), if we can't find a range of mapping in the
extents cache, we are calling ext4_ext_map_blocks() to search the real
path and ext4_ext_determine_hole() to determine the hole range. But if
the querying range was partially or completely overlaped by a delalloc
extent, we can't find it in the real extent path, so the returned hole
length could be incorrect.
Fortunately, ext4_ext_put_gap_in_cache() have already handle delalloc
extent, but it searches start from the expanded hole_start, doesn't
start from the querying range, so the delalloc extent found could not be
the one that overlaped the querying range, plus, it also didn't adjust
the hole length. Let's just remove ext4_ext_put_gap_in_cache(), handle
delalloc and insert adjusted hole extent in ext4_ext_determine_hole().

The "needed" controls the number of ext4_prealloc_space to discard in
ext4_discard_preallocations. Function ext4_discard_preallocations is
supposed to discard all non-used preallocated blocks when "needed"
is 0 and now ext4_discard_preallocations is always called with "needed"
= 0. Remove unnecessary parameter "needed" and remove all non-used
preallocated spaces in ext4_discard_preallocations to simplify the
code.
Note: If count of non-used preallocated spaces could be more than
UINT_MAX, there was a memory leak as some non-used preallocated
spaces are left ununsed and this commit will fix it. Otherwise,
there is no behavior change.

Above issue is happens when enable bigalloc and inline data feature. As
commit 131294c35ed6 fixed delayed allocation bug in ext4_clu_mapped for
bigalloc + inline. But it only resolved issue when has inline data, if
inline data has been converted to extent(ext4_da_convert_inline_data_to_extent)
before writepages, there is no EXT4_STATE_MAY_INLINE_DATA flag. However
i_data is still store inline data in this scene. Then will trigger UAF
when find extent.
To resolve above issue, there is need to add judge "ext4_has_inline_data(inode)"
in ext4_clu_mapped().

The handling of journalled data in ext4_zero_range() is incomplete. We
do not need to commit running transaction but we rather need to
checkpoint pages with journalled data. If we don't, journal tail can be
advanced beyond transaction containing the journalled data and if we
then crash before committing the transaction doing the zeroing we will
have inconsistent (too old) data in the file. Make sure file pages with
journalled data are properly checkpointed before removing them from the
page cache.

Now that filemap_write_and_wait() makes sure pages with journalled data
are safely on disk, ext4_collapse_range() and ext4_insert_range() do
not need special handling of journalled data.

If ENOMEM fails when the extent is splitting, we need to restore the length
of the split extent.
In the ext4_split_extent_at function, only in ext4_ext_create_new_leaf will
it alloc memory and change the shape of the extent tree,even if an ENOMEM
is returned at this time, the extent tree is still self-consistent, Just
restore the split extent lens in the function ext4_split_extent_at.

When converting files with inline data to extents, delayed allocations
made on a file system created with both the bigalloc and inline options
can result in invalid extent status cache content, incorrect reserved
cluster counts, kernel memory leaks, and potential kernel panics.
With bigalloc, the code that determines whether a block must be
delayed allocated searches the extent tree to see if that block maps
to a previously allocated cluster. If not, the block is delayed
allocated, and otherwise, it isn't. However, if the inline option is
also used, and if the file containing the block is marked as able to
store data inline, there isn't a valid extent tree associated with
the file. The current code in ext4_clu_mapped() calls
ext4_find_extent() to search the non-existent tree for a previously
allocated cluster anyway, which typically finds nothing, as desired.
However, a side effect of the search can be to cache invalid content
from the non-existent tree (garbage) in the extent status tree,
including bogus entries in the pending reservation tree.
To fix this, avoid searching the extent tree when allocating blocks
for bigalloc + inline files that are being converted from inline to
extent mapped.
